Learning objectives
To find out incidencence of mycotic aortal aneurysms in patients attending University hospitals in Mostar and Split.
To determine the most important risk factors.
To determine are we providing our patients with the best treatment protocol and what are theresults of our therapy.
Background
Amycotic aneurysmarising from bacterial infection of the arterial wall.
It can be a common complication of the hematogenous spread of bacterial infection.
William Osler first used the term "mycotic aneurysm" in 1885 to describe a mushroom-shaped aneurysm in a patient with subacute bacterial endocarditis. This may create confusion,
since "mycotic" is typically used to define fungal infections.
The majority of such aneurysms are caused by bacteria and that is reason why Fredric Jarrett proposed term “infected aneurysm’’.
Incidence of mAA is 1% to 2% of...
Findings and procedure details
Our study included 536 patients with aortic aneurysm and 6 of them were diagnosed as mycotic aortic aneurysms.
There were 6 male and all of them were severe smokers.
3 blood cultures were isolated as Salmonella species,
1 culture as Treponema pallidum and 2 cases had no microorganisms isolated.
Patients were treated through EVAR/TEVAR procedures.
Conclusion
A mycotic aneurysm of the aortic remains a life - threatening condition,
especially if the aneurysm has already ruptured.Although the content of the aneurysm sac is considered septic,
as was proved by positive blood cultures,
in situ reconstruction is feasible and,
surprisingly,
was not more closely related to higher morbidity and mortality.
