半乳糖凝聚素-3在心肌梗死后心力衰竭家兔心肌纤维化中的作用及其对心室重构的影响

中文摘要:目的 观察半乳糖凝聚素-3(galectin-3)在心肌梗死后心力衰竭家兔心肌中的表达模式及情况,探讨galectin-3在心力衰竭心肌纤维化的作用及其对心室重构影响,为心力衰竭中纤维化基础和临床研究提供新思路。方法 采用结扎冠状动脉前降支的方法制作家兔心肌梗死后心力衰竭模型。采用数字表法将20只家兔随机分为对照组、实验组。实验组给予开胸结扎冠状动脉前降支、对照组仅给予开胸对照处理。于术前及术后2、4、6周采用心脏超声仪测定实验组及对照组家兔心功能。ELISA法检测实验组与对照组术前及术后2、4、6周血清中galectin-3表达水平。RT-PCR法检测心肌组织galectin-3、胶原纤维Ⅰ及胶原纤维ⅢmRNA的表达并计算胶原蛋白Ⅰ/Ⅲ比值。于术后6周取实验组及对照组心肌组织分析左心室(LV)重量并计算左心室重量/体重比值,即左心室肥厚指数;用Masson法胶原染色,分析心肌纤维化的表达情况。结果 与对照组相比,实验组造模术后2、4、6周EF、EDD、LVPWD差异有统计学意义(P<0.01)。术后2、4、6周实验组血清galectin-3浓度高于对照组及术前浓度(P<0.05)。相关性分析示,血清galectin-3浓度与EF呈负相关(r=-0.84,P=0.009),与EDD呈正相关(r=0.905,P=0.020)。实验组galectin-3、胶原蛋白Ⅰ、胶原蛋白Ⅲ的mRNA表达均明显增高(P<0.01)。实验组中梗死区galectin-3、胶原蛋白Ⅰ、胶原蛋白Ⅲ的mRNA表达皆高于梗死边缘区(P<0.01)。病理学测实验组左心室质量及左心室肥厚指数均明显高于对照组(P<0.01)。光学显微镜下实验组可见心肌细胞排列紊乱,部分心肌细胞坏死、水肿、肥大,心肌纤维增粗。小血管壁增厚并可见周围有大量蓝绿色胶原纤维增生并包绕分割心肌细胞,心肌间质纤维增生、胶原大量沉积并伴有炎性细胞浸润。结论 galectin-3可反映心力衰竭心肌纤维化程度,且参与心室重构的病理生理过程,但其具体机制仍不清楚。

Effect of Galectin-3 on Myocardial Fibrosis in Heart Failure Rabbits after Myocardial Infarction and Its Effect on Ventricular Remodeling
Abstract:Objective To observe the expression pattern of galectin-3 in heart failure rabbits after myocardial infarction and to explore the effect of galectin-3 on myocardial fibrosis and ventricular remodeling. Methods The left anterior descending coronary arterys of the rabbits were ligatured for model preparation. Twenty male rabbits were randomly divided into control group and experimental group. The control group was treated by thoracotomy. Cardiac function were detected by cardiac uhrasonography before and after 2, 4, 6 weeks. Serum samples were collected and galectin-3 level of the samples were further tested with ELISA before and after 2, 4, 6 weeks. The expression of myocardial fibrosis was detected by Masson staining in the experimental group and the control group at 6 weeks after operation. galectin-3 and collagenⅠ, collagenⅢ mRNA level of myocardial of the infracted zone were determinated via Real-Time PCR after 6 weeks operation. Calculate the ratio of collagenⅠand collagenⅢ. Left ventricular mass were analysis and left ventricular hypertrophy index was calculated. Results Compared with the control group, the EF, EDD and LVPWD of the experimental group at 2, 4 and 6 weeks after operation were significantly different(P<0.01).Serum galectin-3 level of the experimental group more than control group (P<0.05). correlation analysis showed that serum galectin-3 level was negatively correlated with EF (r=-0.84, P=0.009), and positively correlated with EDD(r=0.905,P=0.020). The mRNA expression of galectin-3, collagen Ⅰ and collagenⅢ were significantly increased in the experimental group(P<0.01).The mRNA expression of galectin-3, collagen Ⅰ and collagenⅢ were all higher in the experimental group than that in the non-infarcted area (P<0.01).The left ventricular mass and left ventricular hypertrophy index of the experimental group were significantly higher than control group (P<0.01). Under the optical microscope, the myocardial cells in the experimental group were disordered, necrosis, edema, hypertrophy and thickening of myocardial fibers. Microvascular wall thickening and visible around a large number of blue-green collagen fibers and surrounded by segmented myocardial cells, myocardial interstitial fibrosis, collagen deposition and accompanied by a large number of inflammatory cell infiltration. Conclusion Galectin-3 reflects the degree of myocardial fibrosis in heart failure, and is involved in the pathophysiology of ventricular remodeling, but the mechanism remains unclear.
