改善學齡前期病童靜脈注射過程疼痛行為反應之專案

Improved Preschool-age Children Behavioral Responses to Pain during Intravenous Cannulation

摘要


靜脈注射是引起學齡前期病童出現疼痛行為反應的原因之一,本兒科病房發現95.31%的學齡前期病童因此出現疼痛行為反應,其中重度疼痛行為反應者佔79.69%。為改善學齡前期病童靜脈注射過程疼痛行為反應,經現況調查學齡前期病童出現重度疼痛行為反應的導因為:曾被多次注射及約束等不良的注射經驗、護理人員在靜脈注射前準備溝通不足及注射技能與知能訓練缺乏、治療室環境佈置單調。專案小組運用實證結果,制訂小兒靜脈注射照護規範、舉行小兒靜脈注射知能及實務操作在職教育、採用量身訂製固定肢體合適的輔具及重新規劃護理臨床作業環境。經改善對策實施後,靜脈注射過程重度疼痛行為反應下降至15.45%,達專案目的。

A study in a pediatric ward found that 95.31% of preschool children presented pain response during intravenous (l.V.) cannulation procedure, and among them, 79.69% were in severe pain. The causes included: 1) unpleasant experience, such as physical immobilization and multiple attempts, 2) nurses did not provide adequate communication to prepare children, 3) nurses lacked skills, knowledge and training in the procedure, and 4) lack of a child-friendly infusion treatment environment. Interventions included development of guidelines for administration of intravenous medications in pediatric patients, educational programs to improve nursing knowledge and clinical skills on pediatric l.V. administration, use of age appropriate immobilizers during insertion procedure, and a renovated child-friendly infusion room. The study of children's pain reactions during intravenous I.V. procedures post implementation of the strategies reduced to 15.45%.
