Абдоминальная боль у беременных: современные возможности дифференциальной диагностики и купирования

В статье показаны основные причины функциональной и органической абдоминальной боли и ее специфические виды при беременности. Автор представила тактику ведения пациенток, особенности интерпретации лабораторных данных при подозрении на острую хирургическую патологию и «тревожные признаки» при абдоминальной боли у беременных. Она подчеркнула значение методов визуализации при дифференциальной диагностике синдрома «острого живота» в гестационном периоде. Обоснована безопасность и необходимость проведения магнитно-резонансной томографии пациенткам с упорной абдоминальной болью. Обсужден клинический случай паралитической кишечной непроходимости у беременной. Проведен анализ лекарственной терапии при абдоминальной боли с учетом преобладания ожидаемой пользы для матери над потенциальным риском для плода. Автор продемонстрировала, что купирование боли в животе опирается на консервативный подход из-за отсутствия доказательной базы по безопасности лекарственных средств для плода и клинических рекомендаций по ведению беременных с абдоминальной патологией. Она акцентировала необходимость юридического оформления назначений магнитно-резонансной и компьютерной томографии, а также фармакотерапии вне инструкции при гестации.
