Liver echinococcosis with transdiaphragmatic spread to the lung is a rare entity, which necessitates an early surgical approach. We present a case of recurrent liver echinococcosis with transdiaphragmatic evolution due to necrosis of the right diaphragmatic dome, causing right pleural and lung echinococcosis. The patient was successfully operated with right thoracophrenotomy, echinococcectomy, lung decortication, and partial pleurectomy.
