ALT ÇENE BÜYÜK AZI DİŞLERİNDE RADİKS ENTOMOLARİS VE RADİKS PARAMOLARİS İLE C ŞEKİLLİ KANAL VARLIĞININ KONİK IŞINLI BİLGİSAYARLI TOMOGRAFİ İLE DEĞERLENDİRİLMESİ

Abstract

Amaç: Bu çalışmanın amacı, konik ışınlı bilgisayarlı tomografi (KIBT) kullanarak alt çene büyük azı dişlerinde radiks entomolaris (RE), radiks paramolaris (RP) ile C şekilli kanal varlığını değerlendirmek ve morfolojik özelliklerini ortaya koymaktır.
Gereç ve Yöntem: Çalışmada 177 hastanın (85 kadın, 92 erkek) tomografi görüntüleri retrospektif olarak incelenmiş, 242 alt birinci ve 285 alt ikinci büyük azı dişi ile çalışma grubu oluşturulmuştur. İncelenen dişlerde, üç kök ve C şekilli kanal varlığının cinsiyet, sağ ve sol taraf, birinci ve ikinci büyük azılar arasındaki dağılımı tespit edilmiştir. Ayrıca üçüncü kökün servikal bölgesinin konumu, bukkolingual yöndeki eğimi ve ilişkili olduğu köke göre uzunluğu da belirlenmiştir.
Bulgular: Alt birinci büyük azı dişlerinin %1.65’inde RE, %0.41’inde RP; ikinci büyük azı dişlerinin %0.35’inde RE, %0.70’inde RP tespit edilmiştir. Alt birinci büyük azı dişlerinde gözlenen RE’lerin servikal bölgesinin konumu değerlendirildiğinde kökün genellikle mezialde yer aldığı (Tip C) ve distal köke göre daha kısa olduğu gözlenmiştir. C şekilli kanal alt birinci büyük azı dişlerinde %0.41, ikinci büyük azı dişlerinde ise %7.72 oranında tespit edilmiştir. Üç kök ve C şekilli kanal varlığı ayrı ayrı değerlendirildiğinde cinsiyetler arasında hem birinci hem de ikinci büyük azı dişlerinde istatistiksel olarak anlamlı bir fark bulunmamıştır (p>0.05).
Sonuç: Üç kök ve C şekilli kanal, Türk toplumunda düşük sıklıkta görülse de klinisyenlerin bu anatomik farklılıklar hakkında bilgi sahibi olması ve gerektiğinde bu farklılıkları KIBT kullanarak tespit edebilmeleri, kök kanal tedavisinin başarısını olumlu yönde etkileyecektir.
