Stellenwert der Lyse bei akuter Lungenembolie

Einleitung

Die Mortalität der akuten Lungenembolie beträgt etwa 30%. Allein in der ersten Stunde nach dem Ereignis sterben 10% der Patienten. Weil die akute Lungenembolie außerdem zu den häufigen Erkrankungen gehört, besteht ein dringender Bedarf zu einer Verbesserung der Therapie. Eine wichtige Option stellt die frühzeitige Lysetherapie dar. Bereits die Erstbeschreibung aus dem Jahre 1964 hatte das erwarten lassen [1]. Es gibt dennoch eine aktuelle Diskussion darüber, ob die Lyse bei Lungenembolie eine gesicherte Indikation hat, oder nicht. Die Datenlage zu diesem Thema ist durchaus kontrovers und die Lager spalten sich in überzeugte Befürworter und überzeugte Gegner der Lyse. Wieso ist die Literatur so kontrovers und warum ist es so schwierig, eine klare Antwort auf die Frage zu geben, ob und wann eine Lysetherapie bei Lungenembolie indiziert ist?
