Die Erdheim-Chester-Krankheit als Differenzialdiagnose bei beiderseitigem Exophthalmus

Zusammenfassung

Berichtet wird über einen Patienten, der sich mit beiderseitigem Exophthalmus, Sehverschlechterung und periorbitalen Xanthelasmen vorstellte. Differenzialdiagnostisch wurde neben einem Morbus Wegener eine Erdheim-Chester-Erkrankung diskutiert. Dabei handelt es sich um eine seltene systemische Histiocytose unbekannter Ätiologie. Befallen wird vor allem das Skelettsystem mit symmetrischen, osteoklastischen Herden insbesondere im Bereich der langen Röhrenknochen. Selten wurde eine Manifestation im Bereich der Orbita beschrieben mit bilateralen retrobulbären Infiltrationen, Exophthalmus, Motilitätsstörungen, Kompression des N. opticus und periorbitale Xanthelasmen.

Abstract

This report describes the case of a patient who had symmetrical exophthalmos, periorbital xanthelasmas and reduced vision. Next to Wegener’s granulomatosis the differential diagnosis of Erdheim-Chester disease is discussed. It concerns a rare systemic histiocytosis of unknown etiology. Above all, the skeleton system with symmetrical long bone osteosclerosis is affected. Manifestations in the area of the orbit have seldom been reported with bilateral retrobulbar infiltrations, exophthalmos, diplopia, compression of the optic nerve and periorbital xanthelasmas.
