Endoskopische Therapie von Dünndarmläsionen und -prozessen

Die Evaluierung pathologischer Läsionen im Dünndarm stellt nach wie vor ein schwieriges differentialdiagnostisches Problem dar. Zum Nachweis von Dünndarmläsionen und -prozessen stehen neben der klinischen Untersuchung sonographische, radiologische, szintigraphische und endoskopische Verfahren zur Verfügung.

Zur endoskopischen Dünndarmdiagnostik finden heute vor allem die Push-Enteroskopie und die gemeinsam von Chirurgen und Gastroenterologen durchgeführte intraoperative Enteroskopie Verwendung. Die Kapsel-Enteroskopie wird sich möglicherweise in der Zukunft als alternative oder ergänzende Methode etablieren.

Die Push-Enteroskopie erlaubt durch den vorhandenen Arbeitskanal die Gewebeentnahme zur histologischen Diagnostik endoskopisch erhobener Befunde; daneben sind therapeutische Interventionen möglich. Der Nachteil der Push-Enteroskopie ist die geringe Eindringtiefe im Vergleich zu der Kapsel- und Sonden-Enteroskopie. Mit der intraoperativen Enteroskopie ist die komplette endoskopische Evaluierung des gesamten Dünndarms, verbunden mit den geeigneten therapeutischen Maßnahmen, möglich.
