We present a case of a 63-year-old man with a recurrent craniopharyngioma with a 2-month history of visual decline, confusion, urinary incontinence, and left homonymous hemianopsia. Imaging evidenced a large suprasellar, solid/cystic mass with progression. The patient underwent maximum safe resection through an extended endoscopic endonasal approach and a fascia Lata graft for reconstruction, tolerating the procedure well, with gross-total resection of the tumor, without postoperative complications and improvement of cognitive and endocrine function.
