Endoscopic-Assisted Transsinusal Approach to Midline Tumors of the Anterior Skull Base: Step-by-Step Procedure in a Series of 16 Cases

The minimally invasive transsinusal approach to the subfrontal region provides ready expeditious access to the base of the anterior cranial fossa without the need for brain retraction, craniotomy, or nasoorbital osteotomies. It represents an excellent alternative in the surgical access of midline tumors in this region of the skull base, providing excellent direct, safe exposure.

Assisted endoscopic surgery allows reduction in the size of the scar, provides better visualization (optic nerves, ICA, cavernous sinus), and enables better control of total removal. We will describe the step-by-step procedure. Between 2009 and 2011, we performed 16 procedures using the endoscopic-assisted transsinusal approach for anterior skull base tumors: eight tubercular meningiomas, six olfactory groove meningiomas, and two esthesioneuroblastomas (with endoscopic endonasal approach) with no complications such as CSF leak, bad cosmetic result, or infection.
