Wolf’s isotopische Antwort: Lokale kutane B-Zell-Lymphom-Infiltration nach H. Zoster N. Trigeminus 1

Zusammenfassung

Wir berichten über einen 62-jährigen Patienten mit seit zehn Jahren bekannter chronischer lymphatischer Leukämie (CLL), der vier Wochen nach dem Auftreten eines Herpes Zoster im Bereich des Nervus Trigeminus 1 links in diesem Areal multiple erythematöse, plaqueartig konfluierende, papulöse Infiltrate entwickelte. Histologisch zeigte sich ein monotones, noduläres kleinzelliges B-Zell-Infiltrat mit klassischem Phänotyp einer CLL mit einer begleitenden periadnexiellen, teils riesenzelligen histiozytär-resorptiven Entzündung. Wir diagnostizierten einen Fall von Wolf’s isotopischer Antwort mit kutanen Infiltraten des bekannten B-Zell-Lymphoms im Innervationsgebiet des betroffenen Nervs bei stattgehabter H.-Zoster-Infektion.

Abstract

We report on a 62-years old male caucasian patient with chronic lymphatic leukemia (B-CLL), known since ten years, who developed, four weeks after the appearance of a Herpes Zoster in the area of the left N. Trigeminus 1, at this site multiple erythematous plaquelike confluating papulous infiltrations. Histologically monotonous nodular small cell B-lymphoid infiltrates were seen together with a partly giant cell histiocytic-resorptive inflammation predominantly around the adnexes. These findings were interpreted as a case of Wolf’s isotopic response with cutaneous infiltrates of the known B-cell lymphoma in the area of the involved innervations of the recently experienced H. Zoster infection.
