Introduction: We report on ultra long-term results after primary repair of partial atrio-ventricular septal defects (pAVSDs).

Material and methods: Between 1960 and 1977 156 patients (83 female/73 male) underwent surgery for pAVSD at our center. Mean age at time of surgery was 15.9±12.6 years (median 10.9 years). In 97% of patients the pAVSD was repaired with a patch, in 92% of patients the mitral cleft was closed. In 2010, patients were sent a mailed questionnaire asking for medical problems and performance in daily life.

Results: Mean follow-up was 34±16 years (0–50 years). 30-day mortality was 9.6%. Survival rates at 1/10/20/30/40/50 years were 87/83/80/77/76/71%, respectively. 30% of patients underwent redo surgery for residual atrial defects (12%), valve replacement (13%), or unknown reasons (5%). After surgery 86% of patients were at some time employed or worked at home.

Conclusions: Even in the early era of cardiac surgery pAVSD repair was performed with excellent ultra long-term results and reasonable quality of life.
