Advances in TransplantationLiver transplantationFactors Affecting Recurrence and Survival After Liver Transplantation for Hepatocellular Carcinoma
Section snippets
Materials and Methods
Medical records of 986 patients who underwent LT between July 2009 and November 2017 were retrospectively analyzed. There were 191 patients with histologically confirmed HCC. Among these, 4 pediatric patients having HCC on the basis of tyrosinemia were excluded. Demographics, liver disease, and tumor-related data as well as recurrence and survival times were recorded for the remaining 187 adult patients. Patients' status of fitting into Milan or UCSF criteria as well as the presence of
Descriptive Analysis
The average age of the patients was 55.3 years, and the majority of the patients were male (87%, n = 162). Average Model for End-Stage Liver Disease (MELD) score was 12.6 and average serum alpha fetoprotein (AFP) level was 58.6 ng/mL.
In 15 patients (8%), HCC was discovered incidentally after histopathological examination.
Majority of the patients (n = 105, 56%) had only 1 tumor. However, there were 25 patients (13.4%) with 6 or more tumors. The mean total tumor size was 56.5 mm (ranging from 6
Discussion
HCC is a challenging disease for which LT offers the best treatment option for suitable patients [2], [4]. Recurrence further complicates the treatment of this challenging disease and thus patient selection is crucial for acceptable results. Various studies can be used to assess factors affecting outcomes of LT for HCC.
Serum AFP level is one of the most frequently studied prognostic factors in HCC. Several studies suggest that serum AFP level independently predicts prognosis and a cutoff value
