Introduction
La prise en charge chirurgicale des kystes de l’ovaire présumés bénins doit répondre à plusieurs impératifs :

assurer une exérèse complète du kyste ;
- •
limiter le risque de récidive, notamment en présence d’un endométriome ;
- •
prévenir tout risque de dissémination tumorale au cas où le kyste se révélerait malin à l’analyse histologique définitive ;
- •
préserver le maximum de tissu ovarien sain afin de ne pas hypothéquer la fertilité ultérieure.
Ces dernières années ont vu l’émergence de nouvelles techniques chirurgicales (cœlioscopie avec trocart unique, chirurgie robot-assistée), de nouvelles énergies (laser CO2, énergie plasma), et d’alternatives à la chirurgie conventionnelle (sclérothérapies à l’éthanol ou au méthotrexate). Ces innovations permettent-elles d’améliorer significativement la prise en charge des kystes de l’ovaire présumés bénins ? Quels bénéfices peut-on en attendre pour les patientes ?
Nous proposons dans cet article de synthétiser les données publiées concernant la prise en charge chirurgicale des kystes ovariens présumés bénins en nous fondant sur une analyse critique de la littérature médicale.

Méthodologie
La recherche bibliographique a été réalisée à l’aide des moteurs de recherche Pubmed/Medline et Cochrane database. Cette recherche a été limitée aux publications en langue française et anglaise sans limite de temps en date du 1er janvier 2013. Nous avons recherché sur Medline les mots clés suivants : ovarian cyst AND surgery AND treatment OR management.
Les articles ont été sélectionnés sur la base du titre, puis du résumé et enfin de l’article intégral. Ont été exclus les cas cliniques et les
Voies d’abord
La ponction écho-guidée d’un kyste de l’ovaire présumé bénin a fait l’objet de nombreuses publications. En revanche, il n’a pas été retrouvé de publications évaluant la ponction de kystes ovariens sous contrôle scanographique ou remnographique.
La ponction écho-guidée, si elle est réalisée, ne doit être réservée qu’aux patientes ayant un kyste répondant aux critères échographiques de bénignité selon la classification du groupe IOTA [1] avec des marqueurs tumoraux (CA125, ACE, CA19-9) normaux.
Sclérothérapies : éthanol, méthotrexate
La sclérothérapie à l’éthanol est essentiellement proposée dans le traitement des kystes endométriosiques. La technique a été décrite en France par Yazbeck et al. [32]. La ponction écho-guidée par voie vaginale s’effectue sous sédation orale et sous anesthésie locale (bloc para-cervical), au bloc opératoire, en respectant les règles d’asepsie. Techniques opératoires
La kystectomie ovarienne, l’ovariectomie ou l’annexectomie sont les techniques chirurgicales de référence dans la prise en charge des kystes ovariens supposés bénins. Leurs indications respectives sont traitées dans un chapitre spécifique (voir « stratégies thérapeutiques des tumeurs ovariennes présumées bénignes »).
La kystectomie ovarienne est intra-péritonéale dans la plupart des cas. Il n’existe que très peu d’indications à la kystectomie transpariétale. Celle-ci peut être envisagée en
Kystes dermoïdes
La particularité des kystes dermoïdes est représentée par un risque de péritonite chimique évalué entre 0,2 et 0,6 % [66], [74] (NP4). Le taux de rupture des kystes dermoïdes est bien documenté, et pourrait être élevé, jusqu’à 80 % dans certaines études [62], [75], [76], [77], [78], [79], [80] (NP4). Prévention du risque infectieux
L’antibioprophylaxie peropératoire est réalisée par la majorité des auteurs et recommandée par la Société française d’anesthésie et de réanimation (SFAR) [86]. Elle repose sur la céfazoline (2 g IVL dose unique ; si durée > 4 h, réinjecter 1 g), le céfamandole (1,5 g IVL dose unique ; si durée > 2 h, réinjecter 0,75 g) ou le céfuroxime (1,5 g IVL dose unique ; si durée > 2 h, réinjecter 0,75 g). En cas d’allergie, on utilisera l’association clindamycine (600 mg dose unique) gentamicine (5 mg/kg dose unique).
