Introducción
El cruce duodenal (CD) ha demostrado ser el procedimiento quirúrgico más efectivo para el tratamiento de la obesidad mórbida y sus comorbilidades1, 2, 3, 4, 5. Sin embargo, actualmente representa un reducido porcentaje de las cirugías bariátricas realizadas en el mundo, probablemente debido a su complejidad técnica y al riesgo de complicaciones a largo plazo6, 7. Diseño del estudio y población
Estudio descriptivo que incluye a los pacientes intervenidos de SADI-S en uno o dos tiempos entre mayo de 2014 y septiembre de 2019. La cirugía bariátrica se indicó siguiendo los criterios de los National Institutes of Health (NIH)10. El algoritmo terapéutico de nuestra unidad indica las técnicas hipoabsortiva en pacientes con un índice de masa corporal (IMC) ± 45 kg/m2 con comorbilidades asociadas y sin clínica sugestiva de reflujo gastroesofágico (RGE), definida como la existencia de pirosis
Datos demográficos basales y operatorios
Se incluyeron 232 pacientes operados de SADI-S, 192 (82,8%) directamente y 40 (17,2%) tras GV, con un intervalo medio entre ambas cirugías de 34,3 meses (rango 6,2-107,1 meses).
En la tabla 1 se describen las principales características de los pacientes intervenidos de SADI-S en uno o dos tiempos. Discusión
Nuestro estudio de cohortes confirma que la técnica SADI-S, en uno o dos tiempos, es una técnica segura y eficaz en pacientes con un IMC > 45 kg/m2, tanto a nivel ponderal como de control de comorbilidades. Actualmente, ésta es una de las cohortes de referencia con mayor número de pacientes y seguimiento a nivel internacional.

This anastomosis was done in a segment of ileum 300cm from the ICV, a measurement considered a little less effective in terms of weight loss, but nutritionally safer, compared to DS, considering all this segment will function as a common channel (13). The Petersen space was not closed, taking into account that internal hernias in SADI-S are rare (13,14).
