Définition
La définition de la non-prise de greffe n’a pas toujours été standardisée. La reconstitution autologue a ainsi longtemps été considérée comme une perte de greffon secondaire [1] avant d’être classée récemment dans les non-prises de greffe primaires [2], [3], tandis que le dysfonctionnement du greffon (poor graft function) est parfois arbitrairement considéré comme une non-prise de greffe. Les études les plus récentes convergent vers une définition cohérente que nous reprendrons ici (encadré 1).
Maladie hématologique
La maladie hématologique sous-jacente est probablement l’un des facteurs de risque influençant le plus fortement la non-prise de greffe. Les patients greffés pour une maladie non maligne ont ainsi jusqu’à trois fois plus de risque de présenter une non-prise de greffe que ceux traités pour une hémopathie maligne [4]. Conclusions
La non-prise de greffe est une complication rare et sévère de l’allogreffe de cellules souches hématopoïétiques. Cette revue nous aura permis de faire le point sur la définition de cette complication, son incidence ainsi que les principaux facteurs de risque identifiés à la lumière des études les plus récentes.
