Se exponen las principales características clínicas de los puntos gatillo miofasciales, utilizables por los fisioterapeutas para el diagnóstico de syndrome de dolor miofascial. También se enumeran los criterios diagnósticos vigentes en la actualidad.
Abstract
The main clinical features of myofascial trigger points that can be used by physical therapists in the diagnosis of myofascial pain syndrome are explained. Current diagnostic criteria are also listed.

Otic symptoms and myofascial pain syndrome in temporomandibular and sternocleidomastoid musculature: A case study
2023, FisioterapiaEl dolor de oído y la alteración de la audición son motivos de consulta muy frecuentes. El dolor de oído referido puede deberse a alteraciones temporomandibulares y puntos gatillo en la musculatura cervical. La hipoacusia se relaciona con la presencia de puntos gatillo en musculatura cervical y mandibular. Aunque se han observado efectos beneficiosos sobre esta musculatura tras la utilización de terapia manual y punción seca, la literatura al respecto sigue siendo escasa, motivo por que se realizó el actual estudio.
Conocer los efectos de un tratamiento basado en la técnica suiza y punción seca en la musculatura temporomandibular y esternocleidomastoideo a corto plazo sobre el dolor de oído e hipoacusia, en una paciente de 24 años.
Estudio de un caso. Se realizó una valoración previa al tratamiento y otra al finalizar para valorar sus efectos, mediante audiometría y escala visual analógica. Se programó una sesión de 50 min, semanalmente, durante cuatro semanas. Durante el tratamiento se aplicó la técnica suiza y punción seca en cinco músculos de la región mandibular y cervical.
Tras cuatro sesiones de tratamiento se observó una mejora del dolor de la paciente y se evidenció una recuperación de la audición.
Una intervención basada en la técnica suiza y punción seca para el tratamiento del síndrome de dolor miofascial de la musculatura temporomandibular y esternocleidomastoideo puede ser útil para mejorar alteraciones de la audición. Se sugiere la realización de ensayos clínicos aleatorizados que evalúen esta técnica.
Ear pain and hearing impairment is a frequent medical consultation. Referred ear pain may be due to temporomandibular disorders and the presence of trigger points in the cervical musculature. Hearing loss is related to the presence of trigger points in the cervical and mandibular muscles. Although beneficial effects have been observed on this musculature after the use of manual therapy and dry needling, the literature on the matter is still scarce, which is why the current study was carried out.
To know the short-term effects of a treatment based on the Swiss technique and dry needling on the temporomandibular and sternocleidomastoid muscles on ear pain and hearing loss, in a female 24-year-old patient.
Case study. An assessment was carried out before the treatment and another at the end to evaluate its effects. A 50-minute session was held weekly for 4 weeks. During treatment, the Swiss technique was applied to 5 muscles of the mandibular and cervical region.
After 4 treatment sessions an improvement in the patient's pain was observed and a hearing recovery was evidenced.
An intervention based on the Swiss technique and dry needling in the treatment of myofascial pain syndrome of the temporomandibular and sternocleidomastoid muscles may be useful in improving hearing disorders and otalgia. Randomized clinical trials evaluating this technique are suggested.
Influence of the treatment of miofascial trigger points in acute ischaemic stroke patients: Randomized pilot study, double blind
2021, FisioterapiaEvaluar la influencia del tratamiento de los puntos gatillo miofasciales (PGM) en el balance motor y en la autonomía funcional en pacientes con ictus isquémico agudo.
Se incluyeron 22 pacientes con ictus isquémico de menos de cinco días de evolución, con paresia braquial y/o crural en un estudio piloto, aleatorizado, a doble ciego, de tratamiento experimental vs. control. Durante la hospitalización, ambos grupos recibieron la fisioterapia. Adicionalmente, al grupo experimental (GE) se le trataron los PGM detectados en hombro y cadera paréticos y al grupo control (GC) se le aplicó un tratamiento sin efectos sobre los PGM. Fueron registrados al alta y a los 90 días postictus el grado fuerza de las extremidades paréticas mediante la National Institute of Health Stroke Scale, y el grado de autonomía funcional mediante la escala de Rankin modificada.
Se detectó una mejoría tanto en la fuerza de las extremidades paréticas (probabilidad al alta de un mejor balance motor del 75,6% en la extremidad superior y del 69% en la extremidad inferior), como en la autonomía funcional (probabilidad de una mejor funcionalidad del 68%) en el GE comparado con el GC. Estos resultados no se mantuvieron a los 90 días de seguimiento.
El tratamiento de los PGM como complemento al tratamiento de fisioterapia durante el ingreso hospitalario parece mejorar el balance motor y el grado de autonomía funcional al alta. Son necesarios futuros estudios que permitan confirmar nuestros resultados y evaluar el beneficio de una intervención más allá del ingreso hospitalario.
To assess the influence of treatment of MTrPs on motor balance and functional autonomy in patients with acute ischaemic stroke.
22 patients with ischaemic stroke of less than 5 days’ evolution, with brachial and / or leg paresis were included in a randomized, double-blind, pilot study of experimental versus control treatment. During hospitalization, both groups received physiotherapy. In addition, the MTrPs detected in the paretic shoulder and hip were treated in the experimental group (EG). In contrast, the control group (CG) were given treatment without effects on the MTrPs. At the time of hospital discharge and 90 days post-stroke, we recorded the degree of strength of the paretic limbs using the National Institute of Health Stroke Scale, and the degree of functional autonomy using the modified Rankin Scale.
We detected an improvement both in the strength of the paretic limbs (probability at discharge of a better motor balance of 75.6% in the upper limb and 69% in the lower limb) and in functional autonomy (68% probability of better functionality) in the EG compared to the CG. These results were not maintained at 90 days of follow-up.
Treatment of MTrPs as a complement to physiotherapy treatment during hospital admission seems to improve the degree of strength of paretic limbs and the degree of functional autonomy at hospital discharge. Future studies are necessary to confirm our results and evaluate the benefit of an intervention beyond hospital admission.
Usefulness of kinesiotaping in the treatment of myofascial trigger points: systematic review

Se realizó una búsqueda bibliográfica en Pubmed, ScienceDirect, Web of Science, Cinhal, Scopus y Cochrane Library, atendiendo a la siguiente estrategia de búsqueda: (Kinesiotape OR Taping OR Kinesiotaping OR Tape) AND («Trigger points»).
Se seleccionaron aquellos artículos que databan entre 2015 y marzo de 2020, que fuesen ensayos clínicos, que estudiaran la eficacia del kinesiotaping en los puntos gatillo en al menos uno de los grupos de intervención y que tuvieran una puntuación mínima de 5/10 en Escala PEDro.
Los artículos analizados poseen un total de 671 pacientes. Todos los artículos muestran mejorías en los parámetros de dolor, rango de movimiento, calidad de vida y fuerza muscular. Se observan diferencias significativas en las comparaciones intragrupales y en las intergrupales con grupos control, grupos tratados con kinesiotaping placebo (sin tensión). Pero no se hallaron diferencias significativas en las comparaciones con otros métodos de fisioterapia como la punción seca.
El kinesiotaping parece ser una buena opción para el tratamiento del dolor, la limitación del rango de movimiento y las limitaciones en las actividades diarias que genera un punto gatillo miofascial. Sin embargo, otras técnicas resultan mucho más efectivas en estos casos. La calidad metodológica de los estudios es mejorable, por lo que se requiere nuevos ensayos clínicos que aporten evidencias científicas sólidas.
The usefulness and effectiveness of kinesiotaping is evaluated in the treatment of the myofascial trigger points caused by myofascial pain syndrome.
A literature search in Pubmed, ScienceDirect, Web of Science, Cinhal, Scopus and Cochrane Library was performed with the following search strategy: (Kinesiotape OR Tape OR Taping OR Kinesiotaping) AND (“Trigger point”).
Articles dating from 2015 to March 2020 that were clinical trials, that studied the efficacy of kinesiotaping at trigger points in at least one of the intervention groups and that had a minimum score of 5/10 in the PEDro scale were selected.
The analysed articles have a total of 671 patients. All articles show improvement in the parameters of pain, range of motion, quality of life and muscle strength. Significant differences are observed in intragroup and intergroup comparisons with control groups, placebo kinesiotaping (no tension) treated groups. But no significant differences were found in the comparisons with other physiotherapy methods such as dry puncture.
Kinesiotaping seems to be a good option for the treatment of pain, limitation of range of movement and limitations in daily activities that generates a myofascial trigger point. However, other techniques are much more effective in these cases. The methodological quality of the studies could be improved, therefore new clinical trials are required to provide solid scientific evidence.
Miofascial pain syndrome as a possible cause of paresthesia: A case report
2007, Revista Iberoamericana de Fisioterapia y KinesiologiaLa presencia de parestesias en el borde cubital de la mano, cuarto y quinto dedos suele tener su origen en la compresión de la raíz C8 o del nervio cubital. A pesar de ello, existen ocasiones en la que dicha sintomatología puede tener otra causa. Se expone a continuación un caso, en el cual el paciente presentaba, además de dolor cervical y en la cara posterior del hombro, parestesias en las zonas antes mencionadas. Durante la exploración, tanto médica como de fisioterapia, se descartó patología compresiva nerviosa. Sin embargo, en la exploración de fisioterapia, se encontró como causa de las parestesias la activación del punto gatillo miofascial del músculo redondo menor. Éste fue tratado en primer lugar mediante técnicas de compresión y estiramiento, no consiguiéndose su inhibición, para lo cual se recurrió a la utilización de punción seca superficial, seguida de estiramiento. Los síntomas desaparecieron a las 3 sesiones de tratamiento.
The presence of paresthesias in the cubital edge of the hand, fourth and fifth finger usually is related to the compression of the C8 root or the cubital nerve. In spite of it, there are occasions in which these symptoms may have other origin. A case study shows, a patient who presented both cervical and dorsal shoulder pain, as well as paresthesias in the zones aforementioned. During the medical and physical therapy examination, compressive nervous pathology was ruled out. During de physical therapy evaluation, it was found out that the paresthesias were related to the presence of an active Myofascial Trigger Point in the Teres Minor. The treatment consisted of compression techniques and stretching. Afterwards, superficial dry needling followed by stretching was needed to obtain the whole inhibition of the MTP. The symptoms disappeared after three sessions of treatment.
Hip arthroplasty and myofascial pain syndrome. A case study
2006, FisioterapiaLa artroplastia de cadera es una de las sustituciones protésicas más empleadas, ya que consigue una mejora en la calidad de vida del individuo, destacando la disminución del dolor y la mejora de la movilidad y funcionalidad. A pesar de ello, existen ciertas complicaciones que, aunque no muy frecuentes, pueden retrasar la recuperación. Una de ellas es el dolor postoperatorio. Se expone el caso de una paciente que, tras sustitución protésica, presentó un patrón de dolor postoperatorio en la cara lateral del muslo. Tras la exploración, se observó que el dolor estaba causado por la presencia de puntos gatillo miofasciales activos: glúteo menor, parte anterior y punto gatillo miofascial 4 del vasto externo. Después de realizar un tratamiento enfocado a la inactivación de dichos puntos gatillo miofasciales, se consiguió la desaparición de dicho patrón doloroso. Todo esto hace pensar en la posible importancia del síndrome del dolor miofascial en el dolor postoperatorio tras la sustitución protésica de cadera.
Hip arthroplasty results to be one of the protesic replacements more used to obtain an improvement in the quality of life, standing out the decreasing of the pain and the increasing in the mobility and functionality. In spite of this, several complications although not very frequent may exist diminishing this improvement. The postoperative pain is one of them. This case study shows the case of a female patient whom after protesic replacement presented a pattern of postoperative pain over the lateral face of the thigh. After the patient's exploration it was found that the pain was caused by the presence of active myofascial trigger points in the gluteus minor (anterior myofascial trigger point) and in the external vastus lateralis (myofascial trigger points 4). The pattern of pain dissapeared after carrying out a treatment based in the deactivation of the myofascial trigger points. All these facts may relate the possible importance of the myofascial pain syndrome with the postoperative pain after the protesic replacement of the hip.
Presence of myofascial trigger points in patients with acute ischaemic stroke: A cross-sectional descriptive pilot study
2019, FisioterapiaDeterminar la presencia de PGM activos y latentes en 4 músculos de hombro y 4 de cadera de las extremidades paréticas y no paréticas, en pacientes con ictus isquémico agudo, y analizar si existe relación con el grado de fuerza de las propias extremidades paréticas.
Se seleccionó a 22 pacientes con ictus isquémico de menos de 5 días de evolución, ingresados en la Unidad de Ictus de un centro hospitalario, que cursaron con una gravedad neurológica leve o moderada (escala National Institute of Health Stroke Scale 3-15) y paresia braquial o crural, y que, previamente al ingreso, presentaban autonomía funcional o incapacidad leve o muy leve (escala Rankin modificada 0-2). Se registraron los PGM en 4 músculos de fácil acceso palpatorio de cada hombro y 4 de cada cadera, siguiendo los criterios diagnósticos de Simons, Travell & Simons.
Todos los pacientes mostraron PGM latentes en hombro y cadera paréticos, siendo más frecuentes en el infraespinoso (94,4%) y en el glúteo mayor (78,9%). El grado de fuerza de las extremidades paréticas se correlacionó con la presencia de PGM latentes en áreas concretas de ambos músculos.
Los resultados orientan hacia una elevada presencia de PGM, predominantemente latentes, en hombro y cadera de las extremidades paréticas secundarias al ictus isquémico agudo. A su vez, apuntan a la posible correlación entre la propia presencia de PGM latentes y el grado de fuerza de las mismas extremidades paréticas. Sin embargo, son necesarios estudios de mayor tamaño muestral que permitan corroborar lo hallado en el presente estudio piloto.
To determine the presence of active and latent MTrPs in 4 shoulder muscles and 4 hip muscles of the paretic and non-paretic limbs, in patients with acute ischaemic stroke, and to analyse if there is a relationship with the degree of strength of the paretic extremities.
22 patients with ischaemic stroke of less than 5 days’ evolution were selected, admitted to the Stroke Unit of a hospital centre with mild or moderate neurological severity (National Institute of Health Stroke Scale 3-15), brachial and / or leg paresis, and that prior to admission presented functional autonomy or mild or very mild disability (modified Rankin scale 0-2). The MTrPs were recorded in 4 muscles of easy palpable access of each shoulder and 4 of each hip, following the diagnostic criteria of Simons, Travell & Simons.
All the patients showed latent MTrPs in the shoulder and hip pads, being more frequent in the infraspinatus (94.4%) and in the gluteus maximus (78.9%). The degree of strength of the paretic limbs correlated with the presence of latent PGMs in specific areas of both muscles.
The results point towards a high presence of MTrPs, predominantly latent, in the shoulder and hip of the paretic limbs secondary to acute ischaemic stroke. In turn, they suggest a possible correlation between the presence of latent MTrPs and the degree of strength of the same paretic limbs. However, studies of larger sample size are needed to corroborate what was found in this pilot study.
