Post-surgical pyoderma gangrenosum of the vaginal vault associated with ulcerative colitis and Bechet's disease; a case report

Abstract

A case is described in which a patient with ulcerative colitis developed chronic vaginal ulceration around the incision lines after cone biopsy. The ulcers persisted at the vaginal vault after abdominal hysterectomy and removal of a cuff of vagina. Non-specific histological features compatible with pyoderma gangrenosum were found on biopsy. The auto-immune nature of this chronic ulcer is further supported by its rapid response to steroid therapy and the subsequent development of Bechet's syndrome.

    Vaginal PG has been rarely reported in the literature and is often initially mistaken for an infection,2 resulting in ineffective treatment, including antibiotic therapy and/or surgical intervention. Vaginal PG can present after a hysterectomy2 or cone biopsy.3 Importantly, the patient presented in this case report had been treated with ixekizumab for psoriasis, raising the possibility of drug-induced PG. PG in the setting of interleukin (IL) 17 inhibition has been reported in a patient with psoriasis treated with secukinumab.4
