Summary

Objectives

The knowledge and the control of the circuit of medicament by the different actors within a hospital group represent major elements in the conflict against the adverse drug event to the hospital. Indeed, the identification of strong points and points to be ameliorated in this circuit will be able to give rise to the installation of corrective actions, in a context of the improvement of professional practices.

Method

For it, a multidisciplinary group has been delegated by the local drug's committee of the hospital group Cochin–Saint-Vincent-de-Paul (GHCSVP) for elaboration, installation and piloting of an audit on the drug circuit.

Results

Eighteen hospitalization wards were audited. Drug circuit basic knowledge of health care professionals were not above 49% of conformity with the audit guideline. Half of services had a conformity under 50% for the drug related equipment and storage. Majority of services had a conformity between 50 and 79% in accordance with the guideline on observation of the practices of nurses and on valuation of the compliance of the patient's file.

Conclusion-Discussion

Group of strong points and other to ameliorate were identified. Working groups have been settled for the eight major improvement actions management and follow-up.

Objectifs

La connaissance et la maîtrise du circuit du médicament par les différents acteurs au sein d’un groupe hospitalier représentent des éléments majeurs dans la lutte contre l’iatrogénie médicamenteuse évitable à l’hôpital. L’identification de points forts et de points à améliorer dans ce circuit doit donner lieu à la mise en place d’actions correctives dans un contexte d’amélioration des pratiques professionnelles.

Méthode

Un groupe pluridisciplinaire a été mandaté par le comité local du médicament du groupe hospitalier Cochin–Saint-Vincent-de-Paul (GHCSVP) pour l’élaboration et le pilotage d’un audit sur le circuit du médicament.

Résultats

Au total, 18 services ont été audités. Les connaissances fondamentales relatives à ce circuit par les professionnels de santé audités ne dépassait pas 49 % de conformité par rapport au référentiel de l’audit. La moitié des services audités avaient une conformité en dessous de 50 % en termes de d’équipement et de stockage. La majorité des services avait une conformité au référentiel comprise entre 50 et 79 % lors de l’observation des pratiques de l’infirmière et lors de l’évaluation du dossier patient.

Conclusion-Discussion

Un ensemble de points forts et certains points à améliorer ont été identifiés. Des groupes de travail se sont constitués pour le pilotage et le suivi de l’avancement des huit actions d’amélioration qui ont paru prioritaires à mettre en œuvre.
