The Eaton-Lambert syndrome and autoimmune disorders

Abstract

The Eaton-Lambert syndrome is a myasthenic disorder often associated with small cell carcinoma of the lung. We describe a patient with hypothyroidism and pernicious anemia in addition to this syndrome to document its association with diseases having autoimmune concomitants.

    Twenty two had cancer of whom 17 had SCLC (Lambert and Rooke, 1965). In 1972, Guttman et al. proposed an autoimmune basis for LEMS after they encountered a patient with LEMS who had thyroiditis and pernicious anemia, but not cancer (Gutmann et al., 1972). In 1981, Lang et al. confirmed an autoimmune etiology by demonstrating clinical improvement with plasma exchange and immunosuppressive therapy, and reproducing the syndrome in mice by infusion of patient IgG fractions(Lang et al., 1981).

    This latter important finding was attributed variously to interference with impulse conduction in terminal nerve filaments (Brooks, 1954), decreased acetylcholine synthesis (Torda and Wolff, 1947), or decreased release of acetylcholine (Stover et al., 1953). As had Guyton and MacDonald earlier (Guyton and MacDonald, 1947), Stephen Thesleff from Lund, Sweden, likened botulinus poisoning to muscle denervation (Thesleff, 1960), poisoned muscle fibers showing persistent fibrillation potentials (Josefsson and Thesleff, 1961).     LEMS patients with SCLC may have other paraneoplastic syndromes, such as inappropriate ADH secretion (SIADH), sensorimotor neuropathy, or cerebellar degeneration (Kobayashi et al., 1988). Before VGCC antibodies had been demonstrated in LEMS, the early report of a NCA‐LEMS patient with several autoimmune disorders raised the possibility that NCA‐LEMS represented an organ‐specific autoimmune disorder (Gutmann et al., 1972). A review of 227 LEMS cases in the literature found immunological disorders in 6% of CA‐LEMS and 27% of NCA‐LEMS (Wirtz et al., 2002a).
