Résumé
Le carcinome hépatocellulaire est la tumeur maligne la plus fréquente du foie. Il se développe le plus souvent dans le contexte d’une maladie chronique du foie. Les patients à risque d’avoir un carcinome hépatocellulaire doivent être enrôlés dans un programme de surveillance pour que le diagnostic soit fait à un stade précoce. En conséquence les patients avec une cirrhose du foie doivent avoir tous les 6 mois un examen ultrasonographique du foie. Si une lésion apparaît il faut faire des examens diagnostics essentiellement radiologiques. Le traitement du carcinome hépatocellulaire est complexe et doit être conduit par des spécialistes de façon multidisciplinaire. Les patients diagnostiqués à un stade précoce peuvent être guéri. Pour les autres de nouvelles options thérapeutiques existent. L’introduction de traitements immuno-oncologiques offre des chances de survie prolongée.
Zusammenfassung
Das Leberzellkarzinom ist der häufigste bösartige Tumor der Leber. In den meisten Fällen entsteht es im Zusammenhang mit einer chronischen Lebererkrankung. Patienten, die ein Risiko für Leberzellkarzinom aufweisen, sollten einem Früherkennungsprogramm zugeführt werden, damit die Diagnose im Frühstadium erfolgt. Somit sollte bei Patienten mit Leberzirrhose alle 6 Monate eine Lebersonographie durchgeführt werden. Wenn eine Veränderung entdeckt wird, sollte eine weitere Abklärung hauptsächlich durch radiologische Untersuchungen erfolgen. Die Behandlung des Leberzellkarzinoms ist komplex und sollte multidisziplinär von Spezialisten durchgeführt werden. Patienten mit einer Diagnose im Frühstadium können geheilt werden. Für die anderen Patienten existieren neue Therapiemöglichkeiten. Die Einführung von Krebsimmuntherapien eröffnet Chancen für ein längeres Überleben.
Riassunto
Il carcinoma epatocellulare è il tumore maligno più comune del fegato. Nella maggior parte dei casi si sviluppa in associazione a malattie croniche del fegato. I pazienti a rischio di carcinoma epatocellulare dovrebbero essere inclusi in un programma di diagnosi precoce, in modo che la diagnosi possa essere effettuata in uno stadio iniziale. I pazienti con cirrosi epatica dovrebbero pertanto essere sottoposti ad ecografia epatica ogni 6 mesi. Se viene rilevato un cambiamento, dovrebbero essere effettuate ulteriori diagnosi principalmente attraverso esami radiologici. Il trattamento del carcinoma epatocellulare è complesso e dovrebbe essere eseguito da più specialisti con un approccio multidisciplinare. I pazienti con una diagnosi precoce possono essere curati. Per gli altri pazienti esistono nuove opzioni terapeutiche. L’introduzione delle immunoterapie antitumorali apre opportunità per una sopravvivenza più lunga.
