Samenvatting
Nederland telt ongeveer 3,5 miljoen mantelzorgers. Gezien de voortschrijdende vergrijzing en extramuralisering van de zorg, zal dit aantal in de toekomst waarschijnlijk toenemen. Kennis over de relatie tussen het geven van mantelzorg en psychische stoornissen bij de mantelzorger is beperkt. Met gegevens van de tweede meting van de ’Netherlands Mental Health Survey and Incidence Study-2’ (NEMESIS-2), een landelijke cohort studie naar psychische aandoeningen onder volwassen Nederlanders (n=5303; 21-68 jaar) onderzochten wij met een gestructureerd diagnostisch interview de prevalentie van stemmings- en angststoornissen (twee veelvoorkomende psychische stoornissen, hierna aangeduid met emotionele stoornissen). Daarnaast zijn risico-indicatoren voor deze stoornissen bepaald bij mantelzorgers. Mantelzorg werd breed gedefinieerd als het verlenen van enigerlei zorg aan een hulpbehoevende uit het sociale netwerk. Een derde van de respondenten verleende mantelzorg (31,1%), deze zorg varieerde in tijd (≥8 uur/week: 32,1%) en duur (≥1 jaar: 48,7%). Mantelzorgers hadden niet vaker een emotionele stoornis dan andere, vergelijkbare, Nederlanders. Geen betaald werk hebben, zonder partner wonen, beperkte sociale steun ervaren, zorg verlenen aan een eerstegraads familielid, partner of vriend, en mantelzorg bieden in de vorm van emotionele steun verhoogden het risico op een emotionele stoornis onder mantelzorgers. Deze risico-indicatoren kunnen gebruikt worden om preventieprogramma’s af te stemmen op kwetsbare mantelzorgers.
Abstract
Risk-indicators for emotional disorders among informal caregivers. Results from NEMESIS-2
There are approximately 3.5 million informal caregivers in the Netherlands. Due to projected demographic developments and care transitions, these numbers may increase substantially in the future. Knowledge regarding the relationship between informal caregiving and emotional disorders among informal caregivers is lacking. Using data from the second wave of the Netherlands Mental Health Survey and Incidence Study-2 (NEMESIS-2), a nationally representative survey among Dutch adults (n=5,303; 21-68 years old), we investigated the prevalence of mood and anxiety disorders (two common mental disorders, hereafter referred to as emotional disorders) using a structured diagnostic instrument. Also, risk-indicators among informal caregivers were examined. Informal care was defined as giving any unpaid care to a relative or close friend in the past year. One-third of the respondents provided informal care (31.1%), which ranged in time spent (≥8 hours/week: 32.1%) and duration (≥1 year: 48.7%). Informal caregiving was not associated with having any 12-month emotional disorder. Having no paid job, living without a partner, lack of social support, giving care to a first-degree relative, partner or close friend and giving emotional support increased the risk for any emotional disorder among caregivers.
