Introduction
L’adénocarcinome primitif du duodénum est une tumeur rare représentant 0,35% des cancers gastro-intestinaux. Nous nous proposons de rappeler à travers six observations, les aspects cliniques, diagnostiques, thérapeutiques et évolutifs de cette affection.
Patients et méthodes
Il s’agit de six patients (quatre hommes et deux femmes) avec un âge moyen de 49 ans, hospitalisés dans un tableau fait surtout d’épigastralgies et d’altération de l’état général. Tous les patients ont eu une fibroscopie avec biopsie duodénale et une TDM abdominale.
Résultats
La fibroscopie avec biopsie était en faveur d’un adénocarcinome primitif duodénal. Il a été réalisé une duodénopancréatectomie céphalique chez 3 patients pour des tumeurs de DII, une résection duodénale chez 2 patients pour une tumeur de DIII et une tumeur de DIV et une double dérivation chez un patient pour une tumeur de DII.
Conclusion
L’adénocarcinome duodénal est une pathologie rare de bon pronostic si la résection chirurgicale est faite précocement.
L’imagerie en coupes surtout la TDM permet de faire le bilan d’extension locorégional et à distance, permettant ainsi d’orienter la prise en charge thérapeutique.
Abstract
Introduction
Primary adenocarcinoma of the duodenum is a rare tumor, which accounts for less than 0.35% of all gastrointestinal cancers. We report six cases adenocarcinoma of the duodenum.
Patients and methods
These six patients with a mean age of 49 years, presented with atypical pain, which was explored by endoscopy and abdominal computed tomography (CT).
Results
Endoscopy with biopsy was in favor of a primary duodenal adenocarcinoma.
It was conducted a cephalic duodénopancréatectomie in 3 patients and a duodenal resection in 2 patients. The treatment was palliative in one case.
Conclusion
The duodenal adenocarcinoma is a rare disease with good prognosis if surgical resection is done early. Imaging, especially CT enables proper for locoregional and distant staging, necessary for guiding therapeutic management.
