Zusammenfassung
In der vorliegenden Untersuchung wurde die regionale Hirnperfusion mit der 99mTc-HMPAO-SPECT morgens vor und nach einem Schlafentzug gemessen. Die Ergebnisse von 15 Patienten (Alter: 53,8±8,9 Jahre, Hamilton-Score: 21,6±7,0), vor und nach partiellem Schlafentzug (PSD, Weckung: 2.30 Uhr), wurden mit denjenigen von 20 Patienten (Alter: 55,0±12,0 Jahre, Hamilton-Score: 23,3±6,5), mit totalen Schlafentzug (TSD) behandelt, verglichen. Alle Patienten wurden zum Untersuchungszeitpunkt mit Antidepressiva behandelt. 60% der Patienten respondierten auf PSD, 65% auf TSD (Abnahme des Hamilton-Scores>30%). In der multiplen Regressionsanalyse (r=0,79) zeigte sich, daß die orbitofrontale/zinguläre (beta −0,78) die Schlafentzugs-Response vorhersagen kann. Ein vergleichbarer Befund ergab sich in der SPECT-Untersuchung vor und nach TSD (r=0,84) für die orbitofrontale Region vor TSD (beta 1,64). Diese Ergebnisse unterstützen die Annahme, daß bei einer Untergruppe depressiver Patienten eine regionale Hyperperfusion in limbischen Strukturen vor Schlafentzug besteht und auf ein günstiges Ansprechen auf totalen oder partiellen Schlafentzug hinweist.
Summary
The possible predictive value of regional cerebral perfusion patterns with respect to partial (PSD) or total sleep deprivation response was evaluated in 15 major depressive patients (age: 53.8±8.9 years, Hamilton-score: 21.6±7.0) and compared with the results of 20 depressed patients (age: 55.0±12.0 years, Hamilton-score: 23.3±6.5), performing total sleep deprivation. The SPECT studies were performed under antidepressant medication. The morning before and after (at 8 a.m.) Tc-99m-HMPAO-SPECT were performed. The response rate after PSD was 60% and 65% after TSD. Multiple regression analysis (r=0.79) showed that orbitofrontal/basal cingulate perfusion (beta −0.78) PSD could predict change of the Hamilton-scores. Similar findings were noted in the TSD group (r=0.84; orbitofrontal region before TSD: beta 1,64). Our findings support the empirical basis suggesting hyperactive limbic structures being present in a subgroup of depressive patients indicating favourable outcome after either partial or total sleep deprivation.
