Zusammenfassung
Anhand der Fallgeschichte eines Patienten mit bereits zum Zeitpunkt der Erstdiagnose fortgeschrittenem kolorektalen Karzinom werden viszeralchirurgische Therapieoptionen im palliativen Setting diskutiert, insbesondere unter dem Aspekt, dass Entscheidungen akut und auf Basis begrenzt verfügbarer Informationen bei primär schon hohem chirurgischen Workload in der Dienstsituation zu treffen sind. Nach Durchsicht der Literatur und dem festgestellten Mangel an qualitativ hochwertigen Studien zur Therapie der malignen Darmobstruktion bei fortgeschrittener Tumorerkrankung kommt der interdisziplinären und interprofessionellen Entscheidungsfindung, der Risikostratifizierung bezüglich invasiver Eingriffe (z. B. unter Verwendung von Prognosetools) und dem Empowerment des Patienten bezüglich der Entscheidungsfindung eine wesentliche Bedeutung zu.
Summary
Although invasive surgical procedures in cancer patients can possibly cure malignancy, these strategies are potentially harmful by restricting quality of life and lifetime. Surgical therapies in palliative situations have to be carefully considered and well planned for every individual patient. The case report of the following patient suffering from malignant small bowel obstruction caused by local recurrence of colorectal cancer may make aware of difficulties and doubts in decision-making. After research oft the literature on this topic it is obvious that standards of therapy currently do not exist.
