Zusammenfassung

Mit der stetig steigenden Zahl nephrologischer Patienten nimmt auch die der Komplikationen als Folge der Urämie, während der Nierenersatztherapie oder nach Nierentransplantation zu. Bei Nierenkranken ist die Lebenserwartung v. a. aufgrund kardiovaskulärer Erkrankungen deutlich eingeschränkt. Bei Hämodialysepatienten stehen Hyperkaliämie, Überwässerung und Shuntkomplikationen im Vordergrund der Notfallbehandlung. Bedrohliche Komplikationen sind Krampfanfälle, akute Blutungen, Tetraparese infolge Hyperkaliämie und Dyspnoe. Sie erfordern eine sofortige Aufnahme in ein Krankenhaus mit nephrologischem Zentrum. Nach Nierentransplantation kommen akute Abstoßungen oder schwere Infektionen und Sepsis als hauptsächliche Komplikationen vor. Akutdialysekatheter, Dialyseshunts und die Cimino-Fistel sind nur in lebensbedrohlichen Notfällen als Gefäßzugänge zu nutzen.

Abstract

The number of patients with kidney disease continues to increase. Thus, more emergencies associated with uremia, dialysis, or transplantation arise. There is excessive cardiovascular morbidity and mortality in patients with chronic kidney disease. In dialysis patients, hyperkalemia, volume overload, and vascular access complications such as thrombosis or septicemia predominate. The most frequent emergencies specific to kidney transplant patients are acute rejection and severe infection. A dialysis Shaldon catheter, a Demers catheter (atrial cuffed tunneled catheter), a Cimino fistula or an artificial shunt should not be used as a vascular access line except in cases of compromised vital functions. The most alarming symptoms in kidney patients are convulsions, bleeding, tetraplegia (because of hyperkalemia), and dyspnea which usually require immediate admission to the next hospital with nephrological expertise.
