Distale Oberschenkel- mehrfragmentfraktur

Historisch gesehen handelt es sich bei der supra- und diakondylären Oberschenkelmehrfragmentfraktur um eine problematische und komplikationsbehaftete Fraktur, weshalb bis in die 70er Jahre ein konservatives Vorgehen empfohlen wurde. Erst nach dem Vorliegen von geeigneten Implantaten besserten sich die Ergebnisse. Wir berichten von 20 Patienten mit 21 Versorgungen durch eine Fixateur-interne-Kondylenplatte. Es handelt sich hierbei um ein ausgewähltes, problematisches Krankengut, größtenteils zusammengesetzt aus Therapieversagern anderer Osteosyntheseverfahren. Bisher kam es zu keinem Implantatversagen. Es konnte rasch eine gute Beweglichkeit im Kniegelenk erreicht werden, wobei 3mal bei vorbestehender hochgradiger Bewegungseinschränkung bzw. Wackelsteifigkeit eine Arthrolyse vorgenommen werden mußte. Obwohl 2 intraoperative Keimabstriche positiv waren, kam es zu keiner Infektproblematik. Durch die einfache Handhabung der Kondylenplatte ist diese problemlos auch minimalinvasiv einsetzbar. Aufgrund unserer Erfahrungen im rekonstruktiven Bereich wird diese Platte auch primär bei der Erstversorgung eingesetzt.

From a historic point of view, supracondylar and intercondylar fractures of the femur are problematic and often complicated by an unsatisfying course. Therefore, until the 1970s, conservative treatment has been recommended. Clinical outcomes started to improve with the development of suitable implants (AO angular condylar plates, condylar plates, and dynamic condylar screws). We report on 21 cases receiving the new internal fixating condylar plates. We first chose complicated courses to study this new type of implant. Until now, we have observed no implant failure. In most cases, we rapidly achieved a good range of knee joint motion. In only in three cases, in which greatly restricted movement of the knee joint was pre-existent, did we have to perform an arthrolysis. We observed no infectious courses, in spite of two positive bacterial swabs taken during operation. Because of its simple handling, the internal fixating condylar plate is suited for minimally invasive surgery. Due to our positive experience with this plate, we have also started to use it for the primary treatment of fractures.
