Versehentliche Fehlpositionierung einer transvenösen Schrittmacher-Elektrode im linken Ventrikel

Zusammenfassung

Wir berichten über den Fall einer unbeabsichtigten Fehlpositionierung einer Schrittmacher-Elektrode im linken Ventrikel. Die Elektrode gelangte über ein offenes Foramen ovale aus dem rechten in das linke Atrium und durch die Mitralklappe in den linken Ventrikel. Nach Sichtung der Literatur zu dieser Komplikation der Schrittmacher-Implantation entschieden wir uns dazu, die Elektrode im linken Ventrikel zu belassen. Unter oraler Antikoagulation war der weitere Verlauf (insgesamt 16 Monate) komplikationsfrei.

Summary

We report the case of inadvertent malpositioning of a pacemaker lead in the left ventricle. The lead went through an open foramen ovale from the right to the left atrium and through the mitral valve into the left ventricle. After a review of the literature, we decided to anticoagulate the patient and leave the electrode in place. During a follow-up period of 16 months, there were no clinical complications.
