Kombinationstherapie mit Remissionsinduktoren bei chronischer Polyarthritis: Update 1996

Summary

Therapy of rheumatoid arthritis with a combination of several disease-modifying drugs aims to better control of the disease than achievable by monotherapy. Subsequent to a paper written two years ago, this publication review studies dealing with combination therapy issued mainly in 1995 and 1996. Most studies deal with MTX as one of the partners. Beneficial results were reported for the combination of methotrexate with antimalarials, cyclosporine or sulfasalazine. The triple combination of methotrexate with hydroxychloroquine and azathioprine is especially promising although the studies presented up to now are still insufficient for its final assessment, due to methodologic problems. Similarly, the value of the combination of sulfasalazine with injectable gold, of sulfasalazine with methotrexate and hydroxychloroquine, or of methotrexate with injectable gold is still uncertain.

Zusammenfassung

Da die monotherapeutischen Behandlungsverfahren der chronischen Polyarthritis noch nicht ihren Ansprüchen genügen, wird vielerorts an Strategien zur Kombination der in Frage kommenden Einzelsubstanzen gearbeitet. Im Anschluß an eine vor zwei Jahren erstellte Literaturübersicht faßt die nachstehende Arbeit vor allem die in den Jahrgängen 1995 und 1996 erschienenen Publikationen zusammen. Die meisten Studien gruppieren sich um MTX, für das in Zweifachkombination mit Chloroquin(-derivaten), Cyclosporin oder Sulfasalazin günstige Ergebnisse berichtet wurden. Insbesondere erscheint die Dreifachkombination von Methotrexat mit Hydroxychloroquin und Azathioprin erfolgversprechend zu sein, auch wenn die vorliegenden Studien wegen methodischer Unschärfen noch keine abschließende Wertung zulassen. Für die Kombination von Sulfasalazin mit intramuskulärem Gold, von Sulfasalazin mit Methotrexat und Hydroxychloroquin sowie von Methotrexat mit intramuskulärem Gold reicht die Datenlage für eine Beurteilung bei weitem noch nicht aus.
