Summary
Poisoning due to tricyclic antidepressant (TCA) ingestion is both a common and serious emergency in Germany. Regarding the widespread activity of TCA, interference with many different organ systems occurs. Besides complications of cerebral and pulmonary function, cardiac arrhythmias are seen as a dangerous effect of TCA intoxication. The mechanisms of pathophysiology in organ damage following TCA intoxication are discussed and an overview on the specific therapy and detoxication is given.
Zusammenfassung
Die Intoxikation mit trizyklischen Antidepressiva (TCA) gehört zu den ebenso häufigen wie schwerwiegenden Vergiftungsnotfällen in Deutschland. Aufgrund einer breitgefächerten Wirkung der TCA kommt es zu oftmals lebensbedrohlichen Beeinträchtigungen vieler Organsysteme. Neben schwerwiegenden zerebralen und pulmonalen Komplikationen sind vor allem Herzrhythmusstörungen im Gefolge einer TCA-Intoxikation gefürchtet. Unter Zugrundelegung der Pathomechanismen wird die spezielle Therapie der Komplikationen der TCA-Intoxikation dargestellt.
