Malignome des Saccus lacrimalis

Zusammenfassung

Hintergrund

Maligne Neoplasien des Saccus lacrimalis sind Seltenheiten in der ophthalmologischen Literatur, für die es nur sehr limitierte diagnostische und therapeutische Handlungsempfehlungen gibt. Wir stellen 5 konsekutive Fälle unserer Klinik aus den Jahren 2006 bis 2009 vor.

Methode

Retrospektive Auswertung aller operativ behandelten Patienten mit Tränenwegserkrankungen der Jahre 2006 bis 2009, Erfassung der malignen Neoplasien und Darstellung des diagnostischen und therapeutischen Vorgehens.

Ergebnisse

Von 1/2006 bis 10/2009 wurden 213 Dakryozystorhinostomien in der Augenklinik Charité, Campus Virchow Klinikum, durchgeführt. Bei 5 Patienten wurde eine intrasakkale maligne Neoplasie mittels Biopsie histologisch nachgewiesen. Keiner der Patienten wies charakteristische Symptome wie blutige Epiphora auf. Bei 2 Patienten lag ein Plattenepithelkarzinom vor, 1 Patient wies ein Adenokarzinom auf. Die anderen Patienten hatten ein Lymphom und ein malignes fibröses Histiozytom. Das therapeutische Vorgehen umfasste die chirurgische Resektion sowie Strahlentherapie. Systemische Manifestationen lagen bei den 4 Patienten mit solidem Tumor nicht vor. In der begrenzten Nachbeobachtungszeit wurden keine Rezidive und keine Metastasierung gesehen.

Schlussfolgerung

In ausgewählten Kollektiven können Malignome des Saccus lacrimalis häufiger als erwartet auftreten. Die klinische Symptomatik erscheint uncharakteristisch.

Abstract

Background

Malignant neoplasms of the lacrimal sac are rare in the ophthalmic literature, for which there are only very limited diagnostic and therapeutic recommendations. We present five consecutive cases of our hospital from 2006 to 2009.

Method

Retrospective analysis of all surgically treated patients with lacrimal duct diseases in the years 2006 to 2009, recording of malignant neoplasms and presentation of diagnostic and therapeutic approach.

Results

From January 2006 until October 2009 we performed 213 dacryocystorhinostomies at the Eye Clinic, Charité Campus Virchow Klinikum. In five patients intrasaccal malignancies were histologically proven by biopsy. None of the patients showed typical symptoms such as bloody epiphora. In two patients, a squamous cell carcinoma was seen, and one patient showed an adenocarcinoma. The other patients had a lymphoma and a malignant fibrous histiocytoma. The therapeutic approach consisted of surgical resection and radiotherapy. Systemic manifestations did not occur in any of the five patients. In the limited follow-up period no recurrences and no metastases were seen.
