Dissoziierte Naheinstellungstrias mit akkommodativem Konvergenzexzess

Zusammenfassung

Wir berichten über einen 8-jährigen Jungen, dessen Naheinstellungstrias nur über das linke Auge auslösbar war. Mit einer Korrektion von RA +1,25/−1,25/169° und LA +1,0/−0,75/24° betrug der Fernvisus R/L 1,0. Der Nahvisus betrug RA 0,5, LA 1,0, mit 3,0 D Nahaddition beidseits 1,0. Stereopsis war bis 100“ nachweisbar (Titmus-Test). Der Prismenabdecktest ergab bei Rechts- (RF) und Linksfixation (LF) in 5 m eine Esophorie von 4°, die in 0,3 m bei RF auf 6° zunahm und bei LF auf 28° dekompensierte. Mit 3,0 D Nahaddition bestand bei RF und LF 3° Esophorie. Die Akkommodation war bei RF beidseits kaum auslösbar, bei LF lag der Flackerpunkt beidseits 15 cm vor dem Auge. Auch die Konvergenzmiosis war nur bei LF auslösbar. Die gesamte Naheinstellungstrias war also nur über das LA auslösbar, das der Junge in der Nähe benutzte, wobei ein akkommodativer Konvergenzexzess auftrat. Durch eine Bifokalbrille wurde Orthotropie mit Random-Dot-Stereopsis in der Nähe erzielt.

Abstract

We report on an 8-year-old boy whose near reflex could be elicited exclusively when the left eye was fixing (LF) but not when the right eye was fixing (RF). With RE +1.25/−1.25/169° and LE +1.0/−0.75/24°, the visual acuity was 1.0 OU at 5 m and RE 0.5, LE 1.0 at 0.3 m improving to 1.0 OU by a near addition of 3.0 D. Stereopsis was 100” (Titmus test). The prism and cover test revealed an esophoria of 4° at 5 m. At 3 m there was an esophoria of 6° (RF) and an esotropia of 28° (LF), compensating to an esophoria of 3° (RF/LF) with a near addition of 3.0 D. Accommodation and the pupillary near reaction (OU) were hardly elicitable during RF. During LF, retinoscopy revealed an accommodation of 8 D (OU) and the pupils constricted normally. Correction by bifocal glasses yielded orthotropia with random dot stereopsis at near.
