Hintergrund
Anorexia nervosa (AN) ist eine psychiatrische Erkrankung, die oftmals zu Osteopenie oder Osteoporose führt. Die Ursache des Knochenverlusts ist unklar und eine multifaktorielle Genese wird vermutet. In der vorliegenden Studie wurden die Auswirkungen der AN auf die Knochendichte untersucht.
Material und Methoden
Bei 20 Patientinnen mit AN (22,9 ± 4,3 Jahren) sowie bei 20 altersvergleichbaren gesunden Probandinnen wurde die Knochendichte mittels Dual-energy X-ray Absorptiometry (DXA) erhoben.
Ergebnisse
Die Knochendichte war an den Wirbelsäulensegmenten L1–L4 (AN-Gruppe 0,99 ± 0,14 g/cm2 vs. Kontrollgruppe 1,20 ± 0,12 g/cm2; p < 0,0001) und an der gesamten Hüfte (AN-Gruppe 0,86 ± 0,13 g/cm2 vs. Kontrollgruppe 1,06 ± 0,11 g/cm2; p < 0,0001) in der AN-Gruppe signifikant vermindert.
Schlussfolgerung
In der AN-Gruppe wurde eine signifikant verminderte Knochendichte im Vergleich zur Kontrollgruppe nachgewiesen. Bei diesen Patientinnen ist eine langfristige Beeinträchtigung der Knochendichte mit verminderter „peak bone mass“ zu erwarten.
Abstract
Introduction
Anorexia nervosa (AN) is a psychiatric disorder and often associated with osteopenia or osteoporosis. The reason for bone loss in AN is still unknown but a multifactorial genesis is suspected. The purpose of this study was to understand the mechanisms involved in bone loss in AN.
Methods
We prospectively measured bone mineral density by dual x-ray absorptiometry in 20 women with AN (mean age ± SD, 22.9 ± 4.3 years) and 20 healthy age-matched controls.
Results
Bone mineral density was significantly reduced in the spine (AN, 0.99 ± 0.14 g/cm2 vs. controls, 1.20 ± 0.12 g/cm2, p < 0.0001) and hips (AN, 0.86 ± 0.13 g/cm2 vs. controls, 1.06 ± 0.11 g/cm2; p < 0.0001) in all patients with AN compared with controls.
Conclusions
Bone mineral density is significantly reduced in patients with AN. Long-term influences on bone mineral density as well as on peak bone mass are expected in these patients.
