Zusammenfassung
Systemische Mykosen sind eine seltene, aber wichtige Differenzialdiagnose bei unklaren importierten Infektionen. Wir berichten über den Fall einer 51-jährigen Deutschen, die während eines Urlaubs in Arizona/USA eine Kokzidioidomykose erworben hatte. Die Erkrankung manifestierte sich Monate später primär als Infiltration der Zunge. Im Rahmen der weiterführenden Diagnostik konnte ein Befall der Lunge festgestellt werden. Aus transbronchial gewonnenen Biopsaten wurde der dimorphe Pilz Coccidioides posadasii als ursächlicher Erreger kultiviert.
Abstract
Systemic mycoses are rare but important differential diagnoses in patients with imported infections. We report the case of a 51-year-old German traveller who acquired coccidioidomycosis during a holiday in Arizona, USA. The disease became apparent several months later primarily as a swelling of the tongue. Subsequent diagnostic investigations revealed infiltration of both lungs. The causal agent Coccidioides posadasii could be cultivated from transbronchial biopsy samples.
