Zusammenfassung

Es wird der Fall einer Milzverletzung im Rahmen einer elektiven Koloskopie mit tödlichem Verlauf berichtet. Hierbei handelt es sich um eine seltene Komplikation, die vornehmlich aufgrund von Zug- bzw. Scherkräften auf das Lig. splenocolicum und Verwachsungen im linken Oberbauch hervorgerufen wird. Im gegenständlichen Fall wurde zunächst von einem ärztlichen Fehlverhalten aufgrund einer durch eine Kolonperforation hervorgerufenen Milzruptur ausgegangen. Ein ärztliches Fehlverhalten konnte jedoch anlässlich der Sektion nicht festgestellt werden.

Abstract

A case of splenic injury following a diagnostic colonoscopy with fatal outcome is reported. Splenic injury is a rare complication of colonoscopy. The most likely mechanism is tension and shearing force on the splenicolic ligament and adhesions in the left upper abdomen. In this case medical malpractice because of perforation of the colon was assumed but could not be confirmed by the autopsy findings.
