Ochronose

Zusammenfassung

Es wird über eine 72 Jahre alt gewordene Patientin berichtet, die seit Jahren unter starken Schmerzen und Bewegungseinschränkungen im Bereich von großen Gelenken und Wirbelsäule litt. Diverse Arztkonsultationen führten zu einem künstlichen Aortenklappen- und Kniegelenksersatz. Im Rahmen dieser Operationen festgestellte schwärzliche Verfärbungen von Aortenintima sowie Gelenkkapsel und -flächen zogen keine weitere Diagnostik nach sich. Eine wirksame Therapie des chronischen Schmerzsyndroms ließ sich nicht erzielen. Die Patientin verstarb letztlich im akuten Herzversagen. Obduktion und nachfolgende mikroskopische Untersuchungen – aber auch die Patientenanamnese – zeigten typische Befunde einer Ochronose. Diese werden dargestellt und u. a. differenzialdiagnostisch diskutiert.

Abstract

The case reports of a 72-year-old woman who had suffered for many years from severe pain and limitation of motion in major joints and the spine. The woman had consulted many doctors, which led to replacement of an aortic valve and a knee joint. During this operation the colour of the aortic intima and cartilage of the knee was found to be black, but there were no following diagnostic examinations. An efficacious therapy of the pain was not found. The patient finally died of acute cardiac failure. The autopsy and subsequent microscopic examinations but also the patients history, revealed typical findings for ochronosis, which are presented and discussed with respect to the differential diagnosis.
