Zusammenfassung
Der Oberflächenersatz am Kniegelenk ist einer der weltweit am häufigsten durchgeführten Eingriffe überhaupt. Trotz der überwiegenden Erfolge sind subjektiv nach Scott et al. (2010) nur etwa 81 % der Patienten mit dem Ergebnis des Eingriffs zufrieden.
Das vorliegende systematische Review vergleicht die postoperative subjektive Patientenzufriedenheit in den Zeiträumen 1990–1999 und 2000–2012 und beschreibt Kausalzusammenhänge sowie Einflussfaktoren. Dabei zeigt sich in den Vergleichszeiträumen ein Anstieg der Zufriedenheit nach Knietotalendoprothesenimplantation von 81,2 % in den 90-er Jahren auf 85,0 % seit dem Jahr 2000. Als Einflussfaktoren auf die postoperative Zufriedenheit werden wiederholt Body Mass Index, Gelenkfunktion, Patientenerwartungen, mentale Einflussfaktoren und Schmerz in den 25 ausgewerteten Publikationen benannt. Einschränkend muss in Ermangelung valider Scores zur Erhebung von subjektiver Zufriedenheit und der unterschiedlichen Studiendesigns auf eine reduzierte Vergleichbarkeit der Aussagen in den untersuchten Publikationen hingewiesen werden.
Abstract
Total knee arthroplasty (TKA) is one of the most common operations in orthopedic surgery worldwide but despite being mainly successful only 81 % of patients are satisfied with the final result. The following systematic review compared patient satisfaction and analyzed the causal connections and influencing factors after TKA between 1990–1999 and 2000–2012. From 1990 to 1999 a total of 81.2 % of patients were satisfied after TKA and in the period 2000–2012 patient satisfaction increased to 85 %. Influencing factors on postoperative satisfaction derived from the 25 publications included in the study were consistently body-mass index, patient expectations, pain, joint function and mental factors. A lack of satisfaction scores and different designs resulted in difficulties in comparing the studies and were subsequently limitations of this study.
