Zusammenfassung
Die postpartale Depression oder Wochenbettdepression wird definiert als depressive Erkrankung mit Beginn innerhalb von 4 Wochen nach der Entbindung. Die Prävalenzrate liegt bei 10–15%, die Symptome werden jedoch oft spät oder gar nicht erkannt. Dargestellt werden Risikofaktoren, Ätiologie, Symptome und differenzialdiagnostische Abgrenzung. Bei allen Müttern sollte ein Screening bezüglich depressiver Symptome 6–8 Wochen postpartal erfolgen. Wichtige diagnostische Verfahren sind das Strukturierte Klinische Interview für DSM-IV und die Edinburgh Postnatal Depression Scale. Die Therapie beinhaltet Psychotherapie, insbesondere die Kognitive Verhaltenstherapie, und medikamentöse Therapie mit trizyklischen Antidepressiva und selektiven Serotonin-Wiederaufnahmehemmern. Weiteren Forschungsbedarf gibt es (1) zur Wirksamkeit unterschiedlicher psychotherapeutischer und medikamentöser Strategien, (2) zu Langzeiteffekten bei mütterlicher Antidepressivaeinnahme für den Säugling und (3) zur adäquaten Prophylaxe für Hochrisikopatientinnen.
Abstract
Postpartum depression is defined as the onset of depressive symptoms during 4 weeks after delivery. Although the prevalence is 10–15%, symptoms are often recognised late or not at all. The present paper outlines the risk factors, etiology, symptoms and differential diagnosis. It is recommended that all women should be screened 6–8 weeks after delivery. Important diagnostic tools are the Structured Clinical Interview for DSM-IV and the Edinburgh Postnatal Depression Scale. Important aspects of psychotherapy, especially cognitive behaviour therapy and pharmacotherapy with tricyclic antidepressants and selective serotonin reuptake inhibitors are described. Future research is required to determine (1) the comparative effectiveness of psychotherapy, pharmacotherapy and a combination of the two, (2) the long-term effects of antidepressant drugs on infants during breastfeeding and (3) adequate prophylaxis of high-risk patients.
