Zusammenfassung
Etwa 3–4% aller Tumoren und tumorähnlichen Läsionen des Skelettsystems sind im Fußskelett lokalisiert. Aufgrund der Lokalisationspräferenz, die viele dieser Läsionen haben, unterscheidet sich das spezifische Spektrum der am Fußskelett vorkommenden Entitäten deutlich vom Spektrum des Gesamtskeletts. Obwohl grundsätzlich fast jede Läsion auch am Fußskelett lokalisiert sein kann, verteilt sich die große Mehrzahl der Läsionen des Fußskeletts auf etwa 10 verschiedene Entitäten.
Die Differenzialdiagnose dieser Läsionen folgt im Grundsatz den Prinzipien, die auch für Läsionen anderer Lokalisation gelten und basiert damit auf der präzisen Analyse der Röntgenmorphologie, des Patientenalters, der genauen Lokalisation und bei bestimmten Differenzialdiagnosen auch der MR-Morphologie.
Im vorliegenden Beitrag wird auf die für das Fußskelett wichtigsten Läsionen und ihre differenzialdiagnostische Zuordnung eingegangen, darüber hinaus auf die Technik der lokalen Ausdehnungsbestimmung.
Abstract
About 3–4% of all tumors and tumor-like lesions of the skeleton are located in the foot. Many of these lesions have a predilection for certain locations, so that the spectrum of entities occurring in the foot differs from the rest of the skeleton. Despite the fact that practically any entity can occur in the foot in rare cases, taken together the ten most frequent lesions make up for the vast majority of tumors and tumor-like lesions of the foot.
The differential diagnosis of these lesions follows the general principles that apply in the rest of the skeleton. It is based on the analysis of the lesion’s X-ray morphology and location, the patient’s age, and in certain entities, the MR morphology.
This article describes the most important tumors and tumor-like lesions of the foot, their differential diagnosis, and the principles of local staging.
