Zusammenfassung
Das Auftreten psychiatrischer Erkrankungen bei Patienten mit HIV-Infektion stellt eine besondere medizinische Herausforderung dar. Hierbei ist zwischen vorbekannten und komorbid auftretenden psychiatrischen Grunderkrankungen einerseits sowie primär organisch bedingten Psychosyndromen aufgrund der HIV-Infektion andererseits zu unterscheiden. Neuropsychiatrische Nebenwirkungen der kombinierten antiretroviralen Therapie (cART) sind insbesondere von Efavirenz (EFV) bekannt. Schwindel, Schlafstörungen, affektive Labilität und verschiedene Formen der Wesensänderung treten bei bis zu 40 % der Patienten auf. Formale oder inhaltliche Denkstörungen sind selten. Die Nebenwirkungen können im Zusammenhang mit Polymorphismen der Zytochrom-P450-Isozyme (insbesondere G516T) stehen, die erhöhte Spiegel von EFV induzieren und sich gehäuft bei Patienten afrikanischer und lateinamerikanischer Herkunft finden. Wir berichten über eine 52-jährige Patientin aus Guinea, die unter dem Verdacht einer HIV-assoziierten neurokognitiven Störung (HAND) sowie zum Ausschluss einer opportunistischen Infektion aufgenommen wurde. Unter Therapie mit EFV, Emtricitabin und Tenofovir war bei der Patientin eine ausgeprägte Wesensänderung mit Depressivität, aggressiven Impulsdurchbrüchen, Halluzinationen und wahnhafter Verkennung aufgetreten. Hierdurch entwickelten sich bei der Patientin psychosoziale Schwierigkeiten und eine Einschränkung der Lebensqualität. Die neurologische Abklärung mittels Magnetresonanztomographie und Liquoranalysen ergab keine erklärenden Befunde. Nach Umstellung von EFV auf Nevirapin, bildeten sich die Symptome innerhalb von 5 Tagen zurück.
Summary
Among patients with human immunodeficiency virus (HIV) infections psychiatric disease poses a particular challenge for caregivers. Neuropsychiatric side effects of efavirenz have been described in up to 40 % of patients showing dizziness, insomnia, unusual dreams, mood instability, personality alterations and thought disorders. In immigrants from Africa and South America these side effects may be related to elevated plasma concentrations of efavirenz due to polymorphisms of cytochrome P450 isozymes (especially G516T). Alleles for these polymorphisms are more frequent in African and South American patients. We report a case of a 52-year-old patient from Guinea who was referred to the department of neurology under the diagnosis of HIV-associated neurocognitive disorder (HAND). Since the start of combined antiretroviral therapy (cART) including efavirenz the patient had suffered severe personality alterations, acoustic and visual hallucinations and delusions which led to discrimination and reduced quality of life. Diagnostic procedures including magnetic resonance imaging (MRT) and spinal fluid analysis resulted in normal values and did not explain the disease. After switching to nevirapin instead of efavirenz the psychotic symptoms disappeared within 5 days.
