Zusammenfassung
Wir stellen den Fall eines 37 Jahre alten, schwerverletzten Patienten vor, der durch einen Sturz aus 6 m Höhe eine bilaterale perilunäre Luxationsfraktur erlitt. Die beidseitige perilunäre Frakturdislokation wurde durch eine herkömmliche Röntgenaufnahme während des Polytraumamanagements gesichert. Die rechte Seite wurde initial geschlossen reponiert und anschließend bei ausgeprägter Instabilität transfixiert. Die linke Seite wurde ebenfalls geschlossen reponiert und mittels Unterarmgips ruhiggestellt. Fünf Tage nach dem initialen Trauma erfolgte die definitive Versorgung der Verletzungen durch eine offene Retention mittels temporärer K-Draht-Fixation und unter Verwendung von Mitek-Ankern. Die K-Drähte wurden 6 Wochen nach erfolgter Operation entfernt. Sechs Monate nach der operativen Versorgung erreichte der Patient einen nahezu vollen Bewegungsumfang in beiden Handgelenken und konnte seinen Beruf als Dachdecker wieder aufnehmen.
Abstract
We present the case of a 37-year-old, multiply injured man who sustained a bilateral perilunar fracture dislocation after a 6 m fall. The fracture dislocations were diagnosed by standard radiographs. The right side was initially treated by closed reduction and external fixation, the left side by closed reduction and splint immobilization. The patient received definite treatment 5 days after the initial injury by open reduction and internal fixation using suture anchors and temporary K-wire fixation which were removed after 6 weeks. The patient achieved almost a full range of motion in both hands and went back to his work as a roofer 5 months after surgery.
