Zusammenfassung
Die Beachtung und Einbeziehung des Haut-Weichteil-Mantels bei der Wahl einer Osteosynthese ist unstrittig. Aufgrund der „natürlichen“ kritischen Weichteile im Bereich des Unterschenkels gilt hier dieser Grundsatz umso mehr und führte zur Entwicklung minimal-invasiver Osteosyntheseformen. Dennoch kann in Ausnahmefällen auch dieses begrenzte Operationstrauma für vorgeschädigte Weichteile zu viel sein.
Wir berichten in diesem Zusammenhang über den Fall einer 62-jährigen Patientin, die sich bei postradiogener Osteopenie eine diaphysäre Unterschenkelfraktur zugezogen hatte. Aufgrund des ebenfalls bestehenden Manschettenradioderms erfolgte zunächst die konservative Therapie, bei der sich allerdings ein zunehmendes Crus valgum entwickelte. Während dies bei „normalem“ Weichteilmantel die ideale Indikation für eine Achskorrektur über einen Verriegelungsnagel darstellt, hätten hier die Verriegelungsinzisionen zu einer nahezu sicheren Weichteilnekrose geführt. Durch Implantation eines expandierbaren Nagels konnte diese Problematik vermieden werden.
Allerdings kam es nach 10 Monaten zu einer Nagelperforation, sodass wir einen retrograden Arthrodesennagel implantierten. Wir stellen anhand dieses Falles die Besonderheiten dieser im mitteleuropäischen Raum selten verwendeten Osteosyntheseform dar und diskutieren andere operative Maßnahmen.
Abstract
Operative procedures on the lower limb demand crucial handling of the surrounding soft tissues. Otherwise skin necrosis may develop and in cases of overlapping to the bone there is a risk of osteitis. Therefore the operative treatment requires an approach which reduces operative trauma to a minimum. However, in some cases even a minimal incision is too traumatic.
The case we present, describes the use of an expandable nail-system to correct a valgus deformity in a lower limb, years after radiation therapy for synovial sarcoma. The distinctiveness in this case is the dystrophic skin after irradiation and the surgical options for correction.
