Zusammenfassung

Dysfunktionelle respiratorische Symptome sind durch rekurrierend oder chronisch auftretende respiratorische Krankheitszeichen charakterisiert. Diese lassen sich nicht durch strukturelle Besonderheiten der Atemwege erklären und können Symptome somatischer Erkrankungen imitieren (somatoforme Atemstörungen). Die frühzeitige Diagnosestellung ist erforderlich, um die Entwicklung von Fehlanpassungen zu vermeiden. Mindestkriterien stehen zur Abgrenzung von somatischen Erkrankungen und für die korrekte Diagnosestellung zur Verfügung. Therapeutische Strategien schließen je nach Krankheitsbild Aufklärung über die Gutartigkeit der Erkrankung, Training von physiologischer Atemtechnik, Selbstkontrolltechniken zur Vermeidung unphysiologischer respiratorischer Symptome und psychologische bzw. psychotherapeutische Unterstützung ein. Untersuchungen zur Effizienz therapeutischer Strategien sind erforderlich.

Abstract

Dysfunctional respiratory symptoms are characterized by recurrent or chronic respiratory signs of disease, which are not explained by structural abnormalities and which may mimic symptoms of somatic diseases (somatoform respiratory disorders). Early diagnosis is necessary to avoid the development of maladaptation. Minimum criteria are available for better discrimination from somatic diseases and for a correct diagnosis. Therapeutic strategies include information about the benign character of the disease, training of normal respiration, self-control techniques to avoid pathological respiratory symptoms and psychological or psychotherapeutic counseling. Studies evaluating the efficacy of therapeutic strategies are needed.
