Zusammenfassung
Die Basis der Typ-2-Diabetes-mellitus-Therapie bei Kindern und Jugendlichen ist die Behandlung der Adipositas durch Ernährungsumstellung und verstärke körperliche Aktivität. Besteht schon ein Insulinmangel oder gelingt die Gewichtsreduktion nicht, ist zusätzlich eine Therapie mit Metformin und/oder Insulin indiziert. Besteht im Rahmen des metabolischen Syndroms dazu eine Dyslipidämie oder eine Hypertonie, ist dies ebenfalls therapiebedürftig. Andere orale Antidiabetika mit unterschiedlichen Wirkprinzipien werden bei Kindern und Jugendlichen selten eingesetzt (Thiazolinedione, Sulfonylharnstoffe, α-Glukosidase-Inhibitoren). Neue Therapiekonzepte berücksichtigen vermehrt die molekularen Grundlagen der Adipositas und des Typ-2-Diabetes-mellitus.
Abstract
Treatment of obesity with dietary changes and increased physical activity is the basis of type 2 diabetes therapy. If a lack of insulin already exists or weight reduction fails, an additional therapy with metformin and/or insulin is indicated. Other indications for this therapy are dyslipidemia or hypertension as part of metabolic syndrome. In adults, further antidiabetic medication with different mechanisms of action (thiazolidinediones, sulphoureas, α-glucosidase inhibitors) are often employed. Newly established therapy concepts take the molecular causes of obesity and type-2-diabetes more into consideration.
