Zusammenfassung
Die pathologische gastroösophageale Refluxaktivität verursacht eine Vielzahl von chronischen Lungenerkrankungen. Der Beweis dieses Zusammenhangs stellt jedoch eine diagnostische Herausforderung dar. Zunächst wurde die klassische pH-Metrie als diagnostisches Standardinstrument eingesetzt, während der letzten Jahre hat sich die über 24 h gemessene 2-Punkt-pH-Metrie bei der Klärung eines Zusammenhangs zwischen Reflux und Lungenerkrankung als überlegen gezeigt und etabliert. Einige Arbeitsgruppen haben Normwerte für den proximalen Messpunkt erstellt, es existiert aber noch keine verbindliche Vereinbarung über Standards der Messung, wie die exakte topographische Positionierung des Sensors, sodass eine Vergleichbarkeit der Daten nur bedingt gegeben ist, da die Position der Elektroden einen erheblichen Einfluss auf die resultierenden Messwerte hat. Die von uns neu errechneten Positionierungsformeln SL(d) für den distalen und SL(p) für den proximalen Messpunkt sollen dazu beitragen, die Sondenlage zu optimieren und Arbeitsgruppen übergreifend einheitliche Vorgehensweisen zu etablieren, sodass eine Vergleichbarkeit der Ergebnisse gewährleistet ist und die Resultate auf der Basis allgemeingültiger Referenzwerte interpretiert werden können.
Abstract
Pathological gastroesophageal reflux activity causes numerous chronic lung diseases. However, proving this correlation poses a diagnostic challenge. Initially, classic pH-metry was employed as the standard diagnostic tool, but in the past few years the 24-h double-probe method for monitoring pH to clarify the relationship between reflux and lung disease has proved to be superior and has become an established procedure. Some working groups have developed standard values for the proximal measuring point. As yet no binding agreement exists on standards for measurement such as the precise topographical position of the sensor so that data are only conditionally comparable since the position of the electrodes significantly influences the resultant measurements. Our newly calculated positioning formulae SL(d) for the distal and SL(p) for the proximal measuring point serve to optimize positioning the sensor and enable working groups to establish comprehensive standardized methods so that the results are comparable and can be interpreted based on universally valid reference values.
