Zusammenfassung
Diabetes und Parodontitis sind weit verbreitete chronische Erkrankungen mit einer zunehmenden Prävalenz in der deutschen Bevölkerung. Zwischen beiden Erkrankungen besteht eine bidirektionale Beziehung: Der Diabetes begünstigt die Entstehung, die Progression und den Schweregrad einer Parodontitis. Die Parodontitis erschwert die glykämische Kontrolle des Diabetes, erhöht das Risiko diabetesassoziierter Komplikationen und möglicherweise sogar dessen Entstehung. Angesichts der vorliegenden Evidenz, die gerade in der Ärzteschaft noch nicht hinreichend kommuniziert worden ist, hat ein Expertengremium bestehend aus 4 Diabetologen und 4 Parodontologen im vorliegenden Konsensusreport die folgenden Fragen bearbeitet: Welchen Einfluss hat Diabetes mellitus auf Parodontitis und parodontale Therapie? Welchen Einfluss hat Parodontitis auf Diabetes mellitus? Was sind die praktischen Konsequenzen, die sich daraus für interdisziplinäre Behandlungsstrategien ableiten? Die Behandlung parodontaler Infektionen sollte integraler Bestandteil des Diabetesmanagements werden, wohingegen die Sicherstellung einer adäquaten glykämischen Einstellung wesentlicher Bestandteil einer erfolgreichen Parodontaltherapie ist.
Abstract
Diabetes and periodontitis are chronic diseases with an increasing prevalence in the German population. There is a bi-directional relationship between both diseases. Diabetes promotes the occurrence, the progression and the severity of periodontitis. Periodontitis complicates the glycemic control of diabetes, increases the risk of diabetes-associated complications and possibly even of its onset. In view of the existing evidence, that is still not sufficiently communicated within the medical community, an expert panel consisting of four diabetologists and four periodontists has addressed the following questions: What is the effect of diabetes mellitus on periodontitis and on periodontal therapy? What is the effect of periodontitis on diabetes mellitus? What are the practical consequences, that result for interdisciplinary treatment strategies? The treatment of periodontal infections should become an integral part of the management of diabetes, whereas glycemic control is a prerequisite for successful periodontal therapy.

Auswertung
2–9 Punkte oder HbA 1c <7,0: leicht erhöhtes Risiko
Aufgrund der vorliegenden Parodontalerkrankung ist ein wenig Vorsicht durchaus angebracht, auch wenn das Risiko für eine Diabeteserkrankung nur leicht erhöht ist. Folgende Punkte sollten beachtet werden, damit sich das Risiko nicht wesentlich erhöht:

Der Parodontalstatus sollte jedes Jahr überprüft werden (PSI). Das Recallintervall muss auf das individuelle Risiko abgestimmt werden.

Gründliche tägliche Mundhygiene, einschließlich der Reinigung der Zahnzwischenräume mit entsprechenden Hilfsmitteln (Zahnseide und/oder Zahnzwischenraumbürsten) ist unverzichtbar. Es sollten solche Mundhygieneprodukte (Zahnpaste/Mundspülung) empfohlen werden, die eine klinisch relevante Antiplaquewirkung und sinnvoller Weise auch einen entzündungshemmenden Effekt besitzen.

Bei Übergewicht sollte der Patient 7% des Körpergewichts abbauen und sich entsprechend den Empfehlungen zur Diabetesvorbeugung eher fettarm ernähren.

Dem Patienten sollte geraten werden, sich an mindestens 5 Tagen der Woche jeweils 30 min so zu bewegen, dass er leicht ins Schwitzen gerät.
-
Ein Blutzuckertest sollte einmal jährlich erfolgen, solange keine Störung des Zuckerstoffwechsels diagnostiziert ist.
10–20 Punkte oder HbA 1c >7,0: erhöhtes Risiko
Auf Basis der bestehenden Parodontalerkrankung liegt ein erhöhtes Risiko für eine Störung des Zuckerstoffwechsels vor:

Der Blutzucker sollte bestimmt werden (vorzugsweise HbA1c), eine Überweisung an den Hausarzt zum Ausschluss eines Diabetes mellitus sollte erfolgen.

Da die parodontale Grunderkrankung einen Risikofaktor für eine Störung des Zuckerstoffwechsels darstellt, sollte besonders auf Entzündungsfreiheit geachtet werden.
-
Die Erhebung eines parodontalen Komplettbefunds wird einmal jährlich empfohlen.
Bei der häuslichen Mundhygiene wie auch den Empfehlungen zur Ernährung und körperlichen Bewegung gelten die obigen Empfehlungen.
Mehr als 20 Punkte: hohes Risiko
Es besteht akuter Handlungsbedarf. Auf Basis der parodontalen Erkrankung, in Verbindung mit den anderen Faktoren, ist das Risiko, dass ein Diabetes vorliegt, sehr groß:

In diesem Fall ist eine Überweisung an den Hausarzt zwecks ausführlicher Labordiagnostik unbedingt empfehlenswert.

Sollte sich der Verdacht einer bereits bestehenden Zuckerkrankheit bestätigen, ist ein interdisziplinärer Austausch zwischen Allgemeinarzt und Zahnarzt sinnvoll.

Auswertung
Normales Risiko
Patient sollte einmal jährlich zum Zahnarzt überwiesen werden zur Erhebung des Parodontalen Screening Index (PSI).
Erhöhtes Risiko
Patient sollte zum Zahnarzt überwiesen werden zur Erhebung eines parodontalen Komplettbefunds. Bei negativem Parodontalbefund ist eine jährliche Überweisung zum PSI zur Überwachung des Parodontalstatus angezeigt.
