Zusammenfassung
Die Helicobacter-pylori-Infektion ist mit zahlreichen gastroduodenalen Erkrankungen assoziiert. Hierzu zählen die Ulkuskrankheit, die funktionelle Dyspepsie (Reizmagen), das MALT-Lymphom und das distale Magenkarzinom. Die Diagnose von H. pylori kann mit nicht-invasiven Tests (13CHarnstoff-Atemtest, Stuhlantigennachweis, Serologie) und invasiven Verfahren (Histologie, Kultur, Urease-Schnelltest) gestellt werden. Bei Erwachsenen sind eine Kultur und Resistenztestung nach einem Versagen der Erstlinientherapie angesagt, wenn eine Kontrollendoskopie erfolgt, vor einer Drittlinientherapie sind sie grundsätzlich durchzuführen. Obligate Indikationen für eine Eradikationstherapie sind das Ulcus ventriculi und duodeni sowie das gastrale MALT-Lymphom. Weitere mögliche Indikationen stellen u. a. die funktionelle Dyspepsie, die Prävention des Magenkarzinoms bei Risikopersonen und die Einleitung einer Dauertherapie mit traditionellen nichtsteroidalen Antiphlogistika dar. Die Erstlinientherapie erfolgt mit der Kombination eines Protonenpumpeninhibitors mit Clarithromycin plus Amoxicillin oder Metronidazol über 7 Tage. In der empirischen Zweitlinientherapie stehen mit Levofloxacin und Rifabutin wirksame Reserveantibiotika zur Verfügung.
Abstract
Helicobacter pylori is associated with various gastroduodenal diseases such as peptic ulcer, functional dyspepsia, MALT lymphoma and distal gastric cancer. Diagnosis of H. pylori can be established by non-invasive (13Curea breath test, stool antigen test, serology) and invasive (histology, rapid urease test, culture) tests. In adults, culture and susceptibility testing should or must be performed after failing of first-line therapy in case of a control endoscopy and before third-line therapy, respectively. Peptic ulcer and gastric MALT lymphoma represent obligatory indications for eradication therapy. Other potential indications are functional dyspepsia, prevention of gastric cancer in individuals being at risk, and before starting treatment with traditional non-steroid antiphlogistics. First-line therapy is performed with a 7-days combination of proton pump inhibitor with clarithromycin and amoxicillin or metronidazole. In second-line therapy levofloxacin and rifabutin are good rescue antibiotics.
