Zusammenfassung
Das Melanom im Stadium der Fernmetastasierung (Stad. IV) ist auch heute noch eine Erkrankung mit in aller Regel infauster Prognose. Obwohl Chemo- oder Chemoimmuntherapien in einer Vielzahl der Fälle partielle Remissionen der Erkrankung bewirken, sind diese Remissionen oft nur kurzdauernd und fast sicher von Rezidiven gefolgt. Um frühzeitige Krankheitsrezidive nach erfolgter Chemo- oder Chemoimmuntherapie zu verhindern, wurden Patienten mit Stadium-IV-Melanomen, die durch die Therapie eine „stabile disease”, partielle oder komplette Remission erfahren hatten, im Anschluß mit einer Kombinationstherapie aus 3×9 Mio. IE IFN-α2a (erste 4 Wochen 5×9 Mio. IE IFN-α2a/Woche) und 3×6 Mio. IE IL-2 s.c./Woche therapiert. Im Vergleich zur unbehandelten Kontrollgruppe zeigte sich in dieser randomisierten Studie, daß die Progressionsrate innerhalb eines Jahres im Vergleich zur Kontrolle von ca. 90% auf 35% abfiel. Auch die Zeit bis zum Auftreten einer Progression wurde signifikant verlängert. Während die mittlere Überlebenszeit in der Kontrollgruppe ca. 25 Wochen betrug, ist die mittlere Überlebenszeit der Behandlungsgruppe nach 15 Monaten noch nicht erreicht. Fernerhin wurde in durchgeführten TNF- ELISPOT-Assays ein signifikanter Anstieg von tumorspezifischen (MAGE-3-reaktiven) zytotoxischen T-Zellen in der Behandlungsgruppe, aber nicht in der Kontrollgruppe, gemessen. Die Immuntherapie im Stadium IV scheint daher sowohl klinisch als auch in vitro meßbar die antitumorale Immunantwort gegen Melanomzellen zu erhöhen. Die von uns „postjuvant” genannte Therapie wird derzeit multizentrisch überprüft.
Summary
Stage IV melanoma is still a disease with poor prognosis. Although modern chemo- or chemoimmunotherapies give high response rates in stage IV patients, remissions are usually followed by fast relapses. In order to avoid early relapses after chemotherapy, patients with stage IV disease and either stable disease or partial or complete remission following therapy were treated with 9 million IU IFNα subcutaneously 5 times weekly and 6 million IU IL-2 subcutaneously twice weekly. Compared with untreated controls, the rate of progression in the treatment group was reduced from 95% to 35%. Also, time to progression was significantly prolonged. Median survival times in the control group were 25 weeks, whereas median survival time in the treatment group has not yet been reached. Furthermore, TNF-ELISPOT assays showed a significant increase in MAGE-3 reactive cytotoxic T-cells in the treatment, but not in the control group. Thus, immunotherapy in stage IV disease seems to prolong survival in melanoma patients.
