Zusammenfassung
Bei einer 50-jährigen Patientin entwickelten sich innerhalb von 12 Jahren ausgedehnte Condylomata acuminata in der Genital- und Perianalregion. Bei der Erstuntersuchung imponierte ein massiver, tumoröser Kondylombefall der gesamten Genitalregion einschließlich des Perineums, der Perianalregion und des angrenzenden Glutealbereichs, der eine Abgrenzung der anatomischen Strukturen nicht zuließ. Proktoskopisch fanden sich keine intraanalen Kondylome. Die STD-Serologie war negativ. Die histologische Untersuchung mehrerer Exzisate ergab keinen Hinweis auf eine maligne Entartung. Im Rahmen der operativen Abtragung in Intubationsnarkose konnte klinisch keine völlige Sanierung erzielt werden. Deshalb behandelten wir immunmodulierend lokal mit Imiquimod (3-mal pro Woche) über 3 Monate und systemisch mit Interferon alpha-2a (subkutan über 5 Monate 3-mal 3 Mio. I.E./Woche). Hierunter kam es zu einer vollständigen Rückbildung der Kondylome bei insgesamt geringer Narbenbildung und vollständigem Erhalt der anatomischen Strukturen. Rezidive traten über einen Nachbeobachtungszeitraum von 1 Jahr nicht auf.
Abstract
A 50-year-old female developed extensive condylomata acuminata in the genitoanal region over a period of 12 years. She presented with multiple, verrucous lesions involving the whole genitoanal area plus the adjacent perineum and gluteal region which made it impossible to identify the anatomical structures. Proctoscopy was normal. Serology for infectious diseases was negative. Several biopsies showed no signs of malignant transformation. Tumor excision under general anaesthesia was incomplete. Therefore, adjunctive therapy with immunomodulatory agents was administered, utilizing local application of imiquimod cream (3 x weekly) for 3 months and subcutaneous injections of interferon-alpha-2a (3 million IU 3x weekly) for 5 months. This treatment regimen resulted in complete remission of all skin lesions with only discrete superficial scarring but no significant loss of anatomical structures or functions. During a 1-year follow-up the patient showed no sign of relapse.
